Clinical observation on the application of conventional high-dose two-line and three-line remedies in patients with peptic ulcer after failure of Hp eradication
Objective:To observe the effect of conventional high-dose two-line and three-line treatment in patients with peptic ulcer after failure of Helicobacter pylori(Hp)eradication.How:Sixty-eight patients with peptic ulcer who had failed to eradicate Hp were selected from our hospital from January 2021 to December 2023 as study objects,and were divided into high-dose dual-dose group(n=34)and three-line remedial group(n=34)according to random number table method.The high-dose dual-dose group was treated with high-dose dual-dose regimen(high-dose amoxicillin combined with esomeprazole).The third-line treatment group administered the third-line treatment regimen(citafloxacin combined with amoxicillin and esomeprazole).The clinical efficacy,eradication rate of Hp,drug resistance rate of Hp strain and adverse drug reactions were compared between the two groups.Results:The total clinical effective rate of the third-line treatment group was 94.12%,which was higher than that of the high-dose dual group(76.47%)(P<0.05).The eradication rate of Hp in the three-line remedy group(85.29%vs 58.82%)was higher than that in the high-dose dual group(P<0.05),and the drug resistance rate of Hp strain(2.94%vs 20.59%)was lower than that in the high-dose dual group(P<0.05).There was no significant difference in the incidence of adverse reactions in the third-line treatment group(20.59%vs 11.76%)compared with the high-dose combination group(P>0.05).Conclusion:Compared with high-dose combination therapy,the three-line remedy can improve the Hp eradication rate,reduce the drug resistance rate and improve the clinical symptoms of patients with peptic ulcer who failed to eradicate Hp while ensuring safety.
