目的:探讨右美托咪定复合丙泊酚、瑞芬太尼麻醉应用于腹腔镜胆囊切除术中,对急性胆囊炎患者血压、心率等的影响.方法:选取本院 2021 年2 月至 2023 年2 月收治的择期拟行腹腔镜胆囊切除术的急性胆囊炎患者 98例作为研究对象.随机将患者分为对照组和观察组,每组各 49 例.对照组采取丙泊酚复合瑞芬太尼麻醉方案,观察组采取右美托咪定复合丙泊酚、瑞芬太尼麻醉方案.分析比较两组的麻醉情况、生命体征[平均动脉压(Mean arterial pressure,MAP)、心率(Heart rate,HR)]、疼痛程度[视觉模拟评分(Visual Analogue Scale/Score,VAS)]、应激反应相关指标[皮质醇(Cortisol,Cor)、白细胞介素-6(Interleukin-6,IL-6)]及不良反应.结果:观察组麻醉苏醒、拔管、自主呼吸恢复时间均明显短于对照组,瑞芬太尼、丙泊酚用量均明显少于对照组(P<0.05).T1~T4时,观察组MAP、HR水平均明显高于对照组(P<0.05);麻醉苏醒时、术后 24 h,观察组VAS评分均明显低于对照组(P<0.05).术后 24 h,观察组COR、IL-6 水平均明显低于对照组(P<0.05).两组的不良反应发生率无显著差异.结论:右美托咪定、丙泊酚、瑞芬太尼复合麻醉应用于腹腔镜胆囊切除术中,能改善麻醉情况,稳定生命体征,缓解疼痛和应激反应,且安全性良好.
Effects of dexmedetomidine combined with propofol and remifentanil on changes of blood pressure and heart rate at different time points in patients with acute cholecystitis
Objective:To investigate the effects of dexmedetomidine combined with propofol and remifentanil anesthesia on blood pressure and heart rate in patients with acute cholecystitis during laparoscopic cholecystectomy.Methods:98 patients with acute cholecystitis who were scheduled to undergo laparoscopic cholecystectomy from February 2021 to February 2023 in our hospital were selected as the study objects,and divided into a control group and an observation group randomly,with 49 patients in each group.The control group was given propofol combined with remifentanil,and the observation group was given dexmedetomidine combined with propofol and remifentanil.the anesthesia status,vital signs[Mean arterial pressure(MAP),Heart rate(HR)],pain level[Visual Analogue Scale/Score(VAS)],stress response related indicators[Cortisol(Cor),Interleukin-6(IL-6)],and adverse reactions were analyzed and compared between the two groups.Results:The recovery time of anesthesia,extubation,and spontaneous respiration in the observation group were significantly shorter than those in the control group,and the dosage of remifentanil and propofol were significantly lower than those in the control group(P<0.05).At T1~T4,the levels of MAP and HR in the observation group were significantly higher than those in the control group(P<0.05);At the time of anesthesia awakening and 24 hours after surgery,the VAS score of the observation group was significantly lower than that of the control group(P<0.05).At 24 h after operation,the levels of COR and IL-6 in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Combined anesthesia including dexmedetomidine,propofol and remifentanil in laparoscopic cholecystectomy can improve anesthesia,stabilize vital signs,relieve pain and stress response,and has good safety.
