Effects of dexmedetomidine combined with propofol and remifentanil on changes of blood pressure and heart rate at different time points in patients with acute cholecystitis
Objective:To investigate the effects of dexmedetomidine combined with propofol and remifentanil anesthesia on blood pressure and heart rate in patients with acute cholecystitis during laparoscopic cholecystectomy.Methods:98 patients with acute cholecystitis who were scheduled to undergo laparoscopic cholecystectomy from February 2021 to February 2023 in our hospital were selected as the study objects,and divided into a control group and an observation group randomly,with 49 patients in each group.The control group was given propofol combined with remifentanil,and the observation group was given dexmedetomidine combined with propofol and remifentanil.the anesthesia status,vital signs[Mean arterial pressure(MAP),Heart rate(HR)],pain level[Visual Analogue Scale/Score(VAS)],stress response related indicators[Cortisol(Cor),Interleukin-6(IL-6)],and adverse reactions were analyzed and compared between the two groups.Results:The recovery time of anesthesia,extubation,and spontaneous respiration in the observation group were significantly shorter than those in the control group,and the dosage of remifentanil and propofol were significantly lower than those in the control group(P<0.05).At T1~T4,the levels of MAP and HR in the observation group were significantly higher than those in the control group(P<0.05);At the time of anesthesia awakening and 24 hours after surgery,the VAS score of the observation group was significantly lower than that of the control group(P<0.05).At 24 h after operation,the levels of COR and IL-6 in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Combined anesthesia including dexmedetomidine,propofol and remifentanil in laparoscopic cholecystectomy can improve anesthesia,stabilize vital signs,relieve pain and stress response,and has good safety.
