Effect of hysteroscopic endometrial resection combined with LNG-IUS in the treatment of patients with abnormal uterine bleeding during perimenopause
Objective:To explore the application effect of hysteroscopic endometrial ablation combined with levonorgestrel intrauterine system(LNG-IUS)on patients with abnormal uterine bleeding during perimenopause(AUBP).Methods:A total of 66 AUBP patients admitted to the hospital were selected from January 2023 to October 2023.33 cases who received LNG-IUS were included in the control group,and 33 cases who adopted hysteroscopic endometrial resection combined with LNG-IUS were enrolled as the combination group.The endometrial thickness,uterine volume,sex hormones levels[luteinizing hormone(LH),follicle stimulating hormone(FSH),progesterone(P)],pictorial blood loss assessment chart(PBAC)score at different time points,recurrence rate and incidence rates of adverse reactions after medication were observed and compared between groups.Results:Before treatment,there were no significant differences in endometrial thickness,uterine volume,PBAC score,and LH,FSH and P levels between the two groups of patients(P>0.05).After 6 months of treatment,the endometrial thickness and uterine volume of the two groups of patients were reduced compared with those before treatment,and the indicators in combined group were significantly thinner or lower than those in control group(P<0.05).The LH,FSH and P levels in both groups of patients were reduced compared to before treatment,and the levels in combined group were significantly lower than those in control group(P<0.05).Ar 1,3,and 6 months after treatment,the PBAC scores in combined group were lower than those in control group(P<0.05).Comparisons of recurrence rate and incidence rates of adverse reactions between the two groups of patients had significant differences(P<0.05).Conclusion:Hysteroscopic endometrial resection combined with LNG-IUS in treating AUBP can reduce endometrial thickness,reduce uterine volume,inhibit the secretions of LH,FSH,and P,effectively reduce the abnormal bleeding volume,and restore normal or even reduce menstrual volume,which has application value.
