目的:探究持续气道正压通气(Continuous positive airway pressure,CPAP)联合肺表面活性物质治疗在早产儿呼吸窘迫综合征(NRDS)治疗中的应用.方法:选取 2022.01-2024.01 我院收治的 60 例NRDS早产儿,随机分为对照组和试验组,各 30 例,对照组给予CPAP治疗,试验组给予CPAP联合肺表面活性物质治疗.治疗 5 d后,比较两组患儿临床疗效、肺功能、血气指标、血清因子水平、并发症.结果:试验组总有效率高于对照组(P<0.05);治疗5 d后,两组患儿氧合指数(Oxygenation index,OI)水平升高,二氧化碳分压(Partial pressure of carbon dioxide,PaCO2)水平降低,试验组和对照组差异有统计学意义(P均<0.05);治疗5 d后,两组患儿呼吸峰流速(Peak expiratory flow,PEF)、达峰时间(Time to reach peak tidal expiratory flow,TPTEF)、达峰容积比(Volume necessary to reach the peak expiratory flow in tidal breathing,VPEF/VE)水平均升高,试验组高于对照组(P均<0.05);治疗5 d后,两组患儿白细胞介素-4(Interleukin,IL-4)水平升高,胱抑素C(Cystatin C,Cys C)水平降低,试验组与对照组差异有统计学意义(P 均<0.05);两组并发症差异无统计学意义(P>0.05).结论:CPAP联合肺表面活性物质可有效治疗NRDS,改善患儿肺功能、血气指标、血清因子水平.
Application of CPAP combined with pulmonary surfactant therapy in treating neonatal respiratory distress syndrome
Objective:To explore the application of CPAP(continuous positive airway pressure)combined with pulmonary surfactant therapy in the treatment of NRDS.Methods:Sixty premature infants with NRDS in the hospital were selected from January 2022 to January 2024,and were randomized into control group and treatment group,with 30 cases in each group.The control group was treated with CPAP,while the treatment group was given CPAP combined with pulmonary surfactant therapy.After 5 days of treatment,the clinical efficacy,pulmonary function,blood gas indicators,serum factors and complications were compared between groups.Results:The treatment group had higher total effective rate(P<0.05).After 5 days of treatment,the oxygenation index(OI)in both groups was risen while the partial pressure of carbon dioxide(PaCO2)was decreased,and the differences between treatment group and control group were statistically significant(all P<0.05).The PEF,TPTEF and VPEF/VE were enhanced in both groups after 5 days of treatment,and the above indicators were higher in treatment group(all P<0.05).The level of interleukin(IL)-4 after 5 days of treatment was risen while the level of cystatin C(Cys C)was declined in the two groups,and there were statistical differences between treatment group and control group(P<0.05).The complications were not statistically different between groups(P>0.05).Conclusion:CPAP combined with pulmonary surfactant can effectively treat NRDS,and improve pulmonary function,blood gas indicators and serum factors.
