摘要
目的:探讨肝硬化合并门静脉血栓患者肠道菌群变化与血小板、凝血酶原时间及白蛋白的关系.方法:选取2020年4月至 2023年4月期间本院收治的具有完整住院资料的肝硬化合并门静脉血栓患者85例作为观察组;同时选择同期在本院体检中心体检的健康体检者80例为对照组.所有受试者均检查肠道菌群和血常规.观察两组患者的肠道菌群、血小板、凝血酶原时间及白蛋白变化情况,并分析肠道菌群变化与血小板、凝血酶原时间及白蛋白的相关性.结果:观察组大肠埃希杆菌、粪肠球菌、乳酸杆菌水平均高于对照组,而双歧杆菌水平低于对照组(P＜0.05);观察组与对照组患者比较,血小板(blood platelet,PLT)、凝血酶原时间(Prothrombin time,PT)水平升高以及白蛋白(Albumin,Alb)水平降低(P＜0.05);大肠埃希杆菌、粪肠球菌、乳酸杆菌均与PLT、PT呈正相关(P＜0.05),双歧杆菌与PLT、PT呈负相关(P＜0.05),大肠埃希杆菌、粪肠球菌、乳酸杆菌均与Alb呈负相关(P＜0.05),双歧杆菌与Alb呈正相关(P＜0.05).结论:肝硬化合并门静脉血栓(Portal vein thrombosis,PVT)患者存在肠道菌群失调,且与血小板、凝血酶原时间及白蛋白具有一定的相关性,有助于对肝硬化合并PVT的诊断和预后评估提供参考.
Abstract
Objective:To investigate the relationship between intestinal flora and platelet,prothrombin time and albumin in patients with cirrhosis complicated with portal vein thrombosis.Methods:From April 2020 to April 2023,85 patients with cirrhosis complicated with portal vein thrombosis were selected as the observation group.At the same time,80 healthy subjects underwent physical examination in the physical examination center of our hospital were selected as the control group.All subjects were examined for intestinal flora and blood routine.The changes of intestinal flora,platelets,prothrombin time and albumin in two groups were observed,and the correlation between the changes of intestinal flora and platelets,prothrombin time and albumin was analyzed.Results:The levels of Escherichia coli,Enterococcus faecalis and Lactobacillus in observation group were higher than those in control group,while the levels of bifidobacterium were lower than those in control group(P<0.05).Compared with control group,PLT and PT levels increased and Alb levels decreased in observation group(P<0.05).Escherichia coli,Enterococcus faecalis and Lactobacillus were positively correlated with PLT and PT(P<0.05),bifidobacterium was negatively correlated with PLT and PT(P<0.05),Escherichia coli,Enterococcus faecalis and Lactobacillus were negatively correlated with Alb(P<0.05),bifidobacterium was positively correlated with Alb(P<0.05).Conclusion:There are intestinal flora disorders in patients with liver cirrhosis complicated with PVT,which are related to platelet,prothrombin time and albumin to a certain extent,which is helpful for the diagnosis and prognosis assessment of liver cirrhosis complicated with PVT.
