微创血肿穿刺引流术与显微镜下血肿清除术治疗高血压性基底节区脑出血的效果及对脑血管痉挛的影响

目的:探讨微创血肿穿刺引流术与显微镜下血肿清除术治疗高血压性基底节区脑出血的疗效及对脑血管痉挛的影响.方法:选取 2020 年 9 月-2023 年 9 月期间本院收治的 76 例高血压性基底节区脑出血患者作为研究对象.随机将患者分为对照组和研究组,每组各38例.对照组予以微创血肿穿刺引流术,研究组予显微镜下血肿清除术.分析比较两组的血肿清除效果、脑血管痉挛的发生率及术后神经功能与日常生活自理能力恢复情况.结果:治疗后,研究组血肿清除率显著高于对照组(P<0.05).研究组的脑血管痉挛发生率显著低于对照组(P<0.05).治疗前,两组美国国力卫生院卒中量表(National institutes of health stroke scale,NIHSS)评分和日常生活活动能力(Modified Barthel Index,MBI评分)均无显著差异(P>0.05);治疗后,研究组的NIHSS评分显著低于对照组,MBI评分显著高于对照组(P<0.05).结论:对高血压性基底节区脑出血患者采用显微镜下血肿清除术进行治疗,能提升血肿清除效果、降低脑血管痉挛的发生率,促进患者术后神经功能的恢复,提高其日常生活自理能力.
Effect of minimally invasive hematoma puncture drainage and microscopic removal of hematoma in the treatment of hypertensive basal ganglia cerebral hemorrhage and its effect on cerebral vasospasm
Objective:To investigate the effect of minimally invasive hematoma puncture drainage and microscopic removal of hematoma on hypertensive basal ganglia cerebral hemorrhage and its effect on cerebral vasospasm.Methods:76 patients with hypertensive basal ganglia cerebral hemorrhage treated in our hospital from September 2020 to September 2023 were selected as the study objects.The patients were randomly divided into control group and study group,with 38 cases in each group.The control group underwent minimally invasive hematoma puncture and drainage,and the study group underwent microscopic hematoma removal.The removal effect of hematoma,the incidence of cerebral vasospasm and postoperative recovery of nerve function and self-care ability of daily life were analyzed and compared between the two groups.Results:After treatment,the clearance rate of hematoma in the study group was significantly higher than that in the control group(P<0.05).The incidence of vasospasm in the study group was significantly lower than that in the control group(P<0.05).Before treatment,the National institutes of health stroke scale(NIHSS)score and Modified Barthel Index of activities of daily living(MBI),There were no significant differences(P>0.05).After treatment,the NIHSS score of the study group was significantly lower than that of the control group,and the MBI score was significantly higher than that of the control group(P<0.05).Conclusion:In the treatment of patients with hypertensive basal ganglia cerebral hemorrhage,the removal of hematoma under the microscope can improve the removal effect of hematoma,reduce the incidence of cerebral vasospasm,promote the recovery of postoperative nerve function,and improve their daily self-care ability.
