Correlation analysis of echocardiographic parameters,serum HMGB1,PTX-3 levels and myocardial injury in patients with sepsis
Objective:To investigate the correlation between echocardiographic parameters,serum high-mobility group box 1(HMGB1),pentraxin 3(PTX-3)levels and myocardial injury in patients with sepsis.Methods:63 patients with sepsis combined with myocardial injury admitted to our hospital from March 2020 to March 2023 were retrospectively selected to be included in the observation group,another 63 patients with simple sepsis during the same period were included in the routine group,and 63 patients with healthy physical examination during the same period were included in the control group.The echocardiographic parameters[left ventricular ejection fraction(LVEF),left ventricular end-systolic posterior wall thickness(LVPWs),left ventricular posterior wall thickness at end diastole(LVPWd),left atrial global ejection fraction(LAEF),left atrial expansion index(LAEI),left atrial stroke volume(LASV)]and serum HMGB1,PTX-3 levels were analyzed and compared in the three groups.Echocardiographic parameters and serum levels of HMGB1,PTX-3,Creatine kinase MB isoenzyme(CK-MB)and Cardiac troponin I(CTnI)were compared at different time points in the observation group,and the correlation between the echocardiographic parameters and serum indexes and CK-MB,CTnI was analyzed.Results:Upon admission,the levels of LVEF,LAEF,and LASV in the three groups in the observation group were lower than those in the sepsis group or the control group,and the levels of the observation group after 2 weeks of treatment was higher than those after 1 week of treatment.The levels of LVPWs,LVPWd,HMGB1,and PTX-3 in the observation group were higher than those in the sepsis group or the control group,and the levels of the observation group after 2 weeks of treatment were lower than those after 1 week of treatment(P<0.05).After 1 and 2 weeks of treatment,the levels of LVEF,LAEF,and LASV in patients with myocardial injury were negatively correlated with CK-MB and CTnI,while the levels of LVPWs,LVPWd,HMGB1,and PTX-3 were positively correlated with CK-MB and CTnI(P<0.05).Conclusion:Echocardiographic parameters and serum levels of HMGB1 and PTX-3 are closely related to the occurrence of myocardial injury in patients with sepsis,and every index is correlated with the degree of myocardial injury,which can provide references for clinical evaluation of the degree of myocardial injury.
