目的:探讨心肌肌钙蛋白I(cardiac troponin,cTnI)、肌酸激酶同工酶(Creatine kinase isoenzymes,CK-MB)、肌红蛋白(Myoglobin,Myo)联合检测对早期急性心肌梗死(Acute myocardial infarction,AMI)的诊断价值.方法:前瞻性研究,选取 2020 年1 月至 2023 年1 月期间于本院就诊的 60 例有早期AMI症状并确诊AMI的患者作为心梗组;选择同期于本院进行体检的60例健康体检者作为对照组.收集两组的基线资料;检测两组的血清cTnI、CK-MB、Myo 水平.分析比较两组的基线资料以及 cTnI、CK-MB、Myo 水平.绘制受试者工作特征(Receiver operator characteristic curve,ROC)曲线评价cTnI、CK-MB、Myo联合检测对早期AMI的诊断价值.结果:两组性别、体质量指数、年龄、糖尿病、高血压、文化程度、饮酒、血糖、血压等基线资料比较均无显著差异(P>0.05).心梗组的cTnI、CK-MB、Myo水平均显著高于对照组,差异有统计学意义(P<0.05).cTnI、CK-MB、Myo单独及联合诊断早期AMI的曲线下面积(Area Under Curve,AUC)分别为 0.845、0.776、0.785、0.929,联合诊断的价值更理想(Z1=1.987,P1=0.044,Z2=3.195,P2=0.001,Z3=2.983,P3=0.003).结论:cTnI、CK-MB、Myo均可单独用于早期AMI的检测,但联合检测,可提高对早期AMI的检出率.
Application value of combined detection of cTnI,CK-MB and Myo in the diagnosis of early acute myocardial infarction
Objective:To investigate the diagnostic value of cardiac troponin(cTnI),creatine kinase isoenzymes(CK-MB)and Myoglobin(Myo)on early Acute myocardial infarction(AMI).Methods:From January 2020 to January 2023,60 patients with early AMI symptoms and diagnosed AMI were selected as the myocardial infarction group.At the same time,60 healthy subjects who underwent physical examination in our hospital during the same period were selected as the control group.Baseline data were collected for both groups.Serum levels of cTnI,CK-MB and Myo were detected in the two groups.Baseline data and cTnI,CK-MB and Myo levels of the two groups were analyzed and compared.Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of cTnI,CK-MB and Myo combined detection in early AMI.Results:There was no significant difference in baseline data between the two groups(P>0.05).The levels of cTnI,CK-MB and Myo in MI group were significantly higher than those in control group(P<0.05).The area under the curve(AUC)of cTnI,CK-MB,Myo alone and combined diagnosis of early AMI were 0.845,0.776,0.785,0.929,respectively.The value of combined diagnosis was better(Z1=1.987,P1=0.044,Z2=3.195,P2=0.001,Z3=2.983,P3=0.003).Conclusion:The combined detection of cTnI,CK-MB and Myo can improve the detection rate of early AMI.
