摘要
目的:探讨抗人乳头瘤病毒(Human papillomavirus,HPV)生物蛋白敷料(简称:抗HPV敷料)联合保妇康栓、重组人干扰素α-2b(Recombinant human interferonα-2b,rhIFNα-2b)凝胶治疗高危HPV感染患者的临床疗效.方法:选取 2021 年 1 月～2022 年 12 月中牟县人民医院收治的 125 例高危HPV感染患者作为研究对象,根据治疗方法不同分为常规组(n=62)和敷料联合组(n=63),常规组采用保妇康栓、rhIFNα-2b凝胶治疗,敷料联合组在常规组基础上联合抗HPV敷料治疗.治疗 3 个疗程后(治疗后),比较两组临床疗效、HPV转阴率、不良反应及治疗前后临床症状积分、阴道微环境因子[(白细胞酯酶(Leukocyte esterase,LE)、过氧化氢(Hydrogen peroxide,H2O2)、唾液酸苷酶(Sialy glycoside enzyme,SNA)]阳性率、血清炎性因子[白细胞介素(Interleukin,IL)-1、IL-6、IL-10、IL-17]、免疫功能[免疫球蛋白(Immunoglobulin,Ig)A、IgG、IgM、表面抗原分化簇(Cluster of differentiation,CD)4+、CD8+].治疗后随访 6 m,比较两组HPV复发情况.结果:敷料联合组总有效率、HPV转阴率高于常规组(P＜0.05);而两组不良反应发生率比较,无显著差异(P＞0.05);治疗后,敷料联合组临床症状积分、阴道微环境因子阳性率、血清IL-1、IL-6、IL-17 水平及CD8+低于常规组,血清IL-10、IgA、IgG、IgM水平及CD4+高于常规组(P＜0.05);治疗后随访 6 m,敷料联合组HPV复发率低于常规组(P＜0.05).结论:抗HPV敷料联合保妇康栓、rhIFNα-2b2 b治疗高危HPV感染患者具有较好的临床疗效,可促进HPV转阴,改善临床症状和阴道微环境,纠正炎性-免疫因子失衡状态,并可降低HPV复发率,且具有较高的安全性.
Abstract
Objective:To investigate the clinical efficacy of anti-human papillomaviru(HPV)bioprotein dressing(anti-HPV dressing)combined with Baofukang suppository and recombinant human interferonα-2b(rhIFN-α2b)gel in the treatment of high-risk HPV infection patients.Methods:A total of 125 patients with high-risk HPV infection admitted to Zhongmou County People's Hospital from January 2021 to December 2022 were selected and divided into conventional group(62 cases)and dressing-combined group(63 cases)according to different treatment methods.The conventional group was treated with Baofukang suppository and rhIFN-α2b gel,and the dressing-combined group was treated with anti-HPV dressings on the basis of the conventional group.After 3 courses of treatment(after treatment),the clinical efficacy,HPV negative conversion rate,adverse reactions,clinical symptom scores,positive rates of vaginal microenvironment factors[(leukocyte esterase,LE),hydrogen peroxide(H2O2),sialy glycoside enzyme(SNA)],serum inflammatory factors[interleukin(IL)-1,IL-6,IL-10,IL-17],immune function[immunoglobulin(Ig)A,IgG,IgM,cluster of differentiation(CD)4+,CD8+]before and after treatment were compared between the two groups.Followed up for 6 months after treatment,the recurrence of HPV was compared between the two groups.Results:The total effective rate and HPV negative conversion rate of dressing-combined group were higher than those of conventional group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After treatment,the clinical symptom score,positive rate of vaginal microenvironment factor,serum levels of IL-1,IL-6,IL-17 and CD8+in dressing-combined group were lower than those in conventional group,while serum levels of IL-10,IgA,IgG,IgM and CD4+were higher than those in conventional group(P<0.05).Followed up for 6 months after treatment,the recurrence rate of HPV in the dressing-combined group was lower than that in the conventional group(P<0.05).Conclusion:Anti-HPV dressing combined with Baofukang suppository and rhIFNα-2b has good clinical efficacy for high-risk HPV infection patients,which can promote HPV negative conversion,improve clinical symptoms and vaginal microenvironment,correct the imbalance of inflammatory and immune factors,and reduce the recurrence rate of HPV,with high safety.
