目的:探讨无创呼吸机对慢阻肺急性发作期(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者临床症状及二氧化碳分压(Partial pressure of carbon dioxide,PaCO2)的影响.方法:选取2021 年1 月至 2023 年 12 月本院收治的 60 例 AECOPD 患者作为研究对象,按照治疗方式分为观察组和对照组,各 30例.对照组有创采用呼吸机治疗,观察组采用无创呼吸机治疗.比较两组治疗疗效.采用改良版英国医院研究委员会呼吸困难量表(Modified medical British research council,mMRC)和急性生理学和慢性健康状况评价II(Acute physiology and chronic health e-valuation II,APACHEII)评分比较两组患者临床症状和健康状况.比较两组患者治疗前后血气指标[动脉血氧分压(Partial pressure of oxygen in artery,PaO2)、PaCO2]和炎症因子[白介素-8(Interleukine-8,IL-8)和基质金属蛋白酶-9(Matrix metalloprotein-9,MMP-9)].比较两组并发症发生率.结果:两组治疗有效率无明显差异(P>0.05).治疗后,观察组PaO2 明显高于对照组,而Mmrc、APACHEII评分、PaCO2、IL-8 和MMP-9 水平、并发症发生率均明显低于对照组(P<0.05).结论:无创呼吸机治疗AECOPD患者与有创呼吸机具有同样的疗效,但是其能更好的恢复血气指标,降低炎症反应,改善临床症状和健康状况,且并发症少.
Effects of non-invasive ventilator on clinical symptoms and partial pressure of carbon dioxide in patients with acute exacerbation of chronic obstructive pulmonary disease
Objective:To explore the effects of non-invasive ventilator on clinical symptoms and partial pressure of carbon dioxide(PaCO2)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:A total of 60 patients with AECOPD admitted to the hospital were enrolled as the research objects between January 2021 and December 2023.According to different treatment methods,they were divided into observation group(n=30,non-invasive ventilator)and control group(n=30,invasive ventilator).The curative effect in the two groups was compared.The clinical symptoms and health status in the two groups were compared by modified medical British research council(mMRC)and acute physiology and chronic health evaluation II(APACHEII).The blood gas indexes[partial pressure of oxygen in artery(PaO2),PaCO2]and inflammatory factors[interleukin-8(IL-8),matrix metalloproteinase-9(MMP-9)]before and after treatment,and incidence of complications were compared between the two groups.Results:There was no significant difference in response rate of treatment between the two groups(P>0.05).After treatment,PaO2 in observation group was significantly higher than that in control group,while scores of mMRC and APACHEII,levels of PaCO2,IL-8 and MMP-9,and incidence of complications were significantly lower than those in control group(P<0.05).Conclusion:Non-invasive ventilator has the same curative effect as invasive ventilator in AECOPD patients.However,the former can better recover blood gas indexes,reduce inflammatory response,improve clinical symptoms and health status,with fewer complications.
