Application value of different detection methods of treponema pallidum in the diagnosis of syphilis
Objective:To evaluate the diagnostic value of enzyme-linked immunosorbent assay(ELISA),gelatin particle agglutination assay(TPPA),and chemiluminescence immunoassay(CLIA)for detecting treponema pallidum in patients with syphilis. Methods:A total of 110 highly suspected syphilis patients admitted to our hospital from January 2023 to December 2023 were selected as the research subjects, and all of them were tested for syphilis spirochete ELISA,TPPA,and CLIA.The comprehensive diagnostic results that meet the guidelines for syphilis diagnosis and treatment of the Centers for Disease Control and Prevention in the United States were used as the gold standard to analyze and compare the detection results and diagnostic efficacy of three different syphilis spirochete detection methods. The detection results of three different syphilis spirochete detection methods for patients with different clinical stages of syphilis were also compared. Results:The three detection methods of treponema pallidum ELISA,TPPA,and CLIA were applied to the diagnosis of syphilis, and there were no significant differences in the misdiagnosis rate, missed diagnosis rate, and consistency with the gold standard detection results(P>0.05).Moreover,there were no significant differences in the positive predictive value, negative predictive value, sensitivity, and specificity between the three treponema pallidum detection methods(P>0.05).Compared with the accuracy of treponema pallidum ELISA and CLIA,there was no significant difference in the accuracy of treponema pallidum TPPA and CLIA(P>0.05).However,the accuracy of treponema pallidum ELISA was significantly lower than that of TPPA(P<0.05).The three treponema pallidum detection methods had no significant differences in the misdiagnosis rate, missed diagnosis rate, and consistency with the gold standard detection results for patients with different clinical stages of syphilis(P>0.05).However,the accuracy of treponema pallidum ELISA was significantly lower than that of TPPA(P<0.05).Conclusion:TPPA,ELISA,and CLIA all have high clinical application value in the diagnosis of syphilis, and among the three,TPPA has the highest accuracy.
