Exploring the diagnostic value of carotid artery color Doppler ultrasound in patients with cerebral infarction and carotid artery stenosis
Objective:To explore the diagnostic value of carotid artery color Doppler ultrasound for carotid stenosis in patients with cerebral infarction.Methods:A retrospective study was conducted on 98 patients with cerebral infarction who were treated in our hospital from August 2021 to August 2023.After admission,all patients underwent carotid artery color Doppler ultrasound and magnetic resonance imaging to observe the degree of carotid artery stenosis.The results of digital subtraction angiography(DSA)were used as the"gold standard"to compare the carotid artery color Doppler ultrasound parameters[common carotid artery peak systolic velocity(PSV),end diastolic velocity(EDV)]in patients with different degrees of carotid artery stenosis.Analyze the correlation between carotid artery color Doppler ultrasound parameters and the degree of carotid artery stenosis.Compare the diagnostic results and diagnostic efficacy of carotid artery color Doppler ultrasound and magnetic resonance imaging for carotid artery stenosis.Analyze the consistency between the diagnostic results of carotid artery stenosis using carotid artery color Doppler ultrasound and magnetic resonance imaging and those obtained through DSA.Results:Comparison of PSV and EDV in patients with different degrees of carotid stenosis:mild stenosis<moderate stenosis<severe stenosis(P<0.05).PSV and EDV were positively correlated with the degree of carotid stenosis in patients with cerebral infarction(P<0.05).Out of 98 patients with cerebral infarction,69 tested positive,and 60 of them were confirmed positive by magnetic resonance imaging.The sensitivity and accuracy of carotid artery color Doppler ultrasound for the diagnosis of carotid artery stenosis were 94.44(68/72)and 94.90(93/98),respectively,which were higher than those of magnetic resonance imaging(MRI)with a sensitivity of 80.56(58/72)and an accuracy of 83.67(82/98),with a missed diagnosis rate of 5.56(4/72),which was lower than that of MRI with a missed diagnosis rate of 19.44(14/72)(P<0.05).The consistency Kappa value between the degree of carotid stenosis in patients with cerebral infarction as assessed by carotid color Doppler ultrasound and DSA was 0.826,and the consistency Kappa value between the degree of carotid stenosis in patients with cerebral infarction as assessed by magnetic resonance imaging and DSA was 0.743.Conclusion:Carotid artery color Doppler ultrasound has high diagnostic value for carotid stenosis in patients with cerebral infarction.
