目的:研究超短波联合主动运动锻炼疗法对缺血性脑卒中(Cerebral ischemic stroke,CIS)后偏瘫患者上肢功能的影响.方法:选择自 2021 年1 月至 2024 年2 月于我院接受康复治疗的脑卒中后偏瘫患者 60 例,随机法分成对照组(30 例,给予常规治疗和主动运动锻炼疗法)与观察组(30 例,额外给予超短波治疗),比较两组治疗前及治疗 4 w 后采用量角器测定的肩关节前屈、后伸、外展活动度,上肢 fugl-meyer 运动功能评定量表(Fugl-meyer assessment,FMA)评分、简易上肢机能检查(Simple test for evaluating hand function,STEF)评分、健康调查简表(36-item short-form,SF-36)评分、一般自我效能量表(General self efficacy scale,GSES)评分、生活质量评分量表(Modified-rankin-scale,MRS)评分,并比较并发症发生情况.结果:治疗后两组肩关节前屈、后伸、外展活动度以及上肢FMA、STEF、SF-36、GSES均升高,且观察组更高(P<0.05),治疗后两组MRS均降低,且观察组更低(P<0.05),观察组并发症发生率低于对照组(P<0.05).结论:超短波联合主动运动锻炼疗法可有效提高CIS偏瘫患者肩关节活动度、上肢功能、自我效能感及生活质量,降低并发症发生率.
Effect of ultra-short wave combined with active exercise therapy on upper limb function in patients with hemiplegia after cerebral ischemic stroke
Objective:To investigate the effect of ultra-short wave combined with active exercise therapy on upper limb function in patients with hemiplegia after cerebral ischemic stroke(CIS).Methods:From January 2021 to February 2024,60 patients with hemiplegia after CIS in hour hospital were selected.They were randomly assigned to the control group(30 cases,active exercise therapy in addition to conventional treatment)and the observation group(30 cases,ultra-short wave therapy on the basis of therapies in the control group).The ranges of anterior shoulder flexion,posterior extension and abduction,upper limb function[Fugl-Meyer Assessment(FMA)]scores,simple test for evaluating hand function(STEF)scores,36-item short-form(SF-36)scores,General Self Efficacy Scale(GSES)scores,Modified Ranking Scale(MRS)score,and the incidence of complications were compared between the two groups.Results:After treatment,there were increases in the ranges of anterior shoulder flexion,posterior extension and abduction,upper limb FMA scores,STEF scores,SF-36 scores and GSES scores in both groups.Moreover,all the above indicators in the observation group were higher than those in the control group(P<0.05).After treatment,MRS scores of both groups decreased,and the observation group had lower scores(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Ultra-short wave combined with active exercise therapy can effectively increases the range of motion of the shoulder in patients with hemiplegia after CIS,improve their upper limb function,self-efficacy and quality of life,and reduce the incidence of complications.
