Influence of bracketless invisible appliance and traditional bracket on periodontal health and gingival crevicular fluid inflammatory factors in orthodontic patients with periodontitis
Objective:To explore the influence of bracketless invisible appliance and traditional bracket on periodontal health and inflammatory factors in gingival crevicular fluid of orthodontic patients with periodontitis.Methods:60 patients with periodontitis orthodontics in the hospital were selected from March 2021 to December 2023 as the research subjects.According to different orthodontic methods,the patients were divided into traditional group(n=27,receiving traditional bracket orthodontics)and invisible group(n=33,receiving bracketless invisible orthodontics).After 6 months of orthodontics,the periodontal health,gingival crevicular fluid inflammatory factors levels and complications of the two groups were compared.Results:Before orthodontics,the periodontal health level in the two groups was similar(P>0.05).After 6 months of orthodontics,the gingival index(GI),clinical attachment loss(CAL),plaque index(PLI)and sulcus bleeding index(SBI)in traditional group were higher than those before orthodontics(P<0.05).There were no statistical differences in GI,CAL,PLI and SBI in invisible group before and after orthodontics(P>0.05),but GI,CAL,PLI and SBI in invisible group were lower than those in traditional group(P<0.05).After 6 months of orthodontics,the levels of tumor necrosis factor(TNF-α),interleukin-1-β(IL-1β)and interleukin-6(IL-6)in both groups were decreased compared with those before orthodontics,and the levels in invisible group were lower than those in traditional group(P<0.05).The total incidence rate of complications in invisible group with 3.03%was lower than 22.22%in traditional group(P<0.05).Conclusion:Compared with traditional bracket,the application of bracketless invisible appliance in orthodontic patients with periodontitis is more effective,which can more effectively maintain periodontal health,improve periodontal inflammation,and reduce complications.
