养阴清热汤加减对阴虚型老年糖尿病肾病的疗效及炎症因子CRP、IL-6的影响

摘要
目的:研究养阴清热汤加减对阴虚型老年糖尿病肾病的疗效及炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)]的影响.方法:将2020年1月～2022年12月医院收治的65阴虚型老年糖尿病肾病按照随机数字表分为中医组(n=33)和西医组(n=32),西医组患者进行常规西医对症治疗,中医组患者在西医组的基础上进行养阴清热汤加减.比较两组治疗效果、中医证候积分、炎症因子[CRP、IL-6、肿瘤坏死因子-α(TNF-α)]、肾功能指标[血肌酐(SCr)、尿素氮(BUN)、膀胱抑素-C(Cys-C)].结果:两组治疗总体有效率相近(P＞0.05),但中医组显效例数多于西医组(P＜0.05).治疗后,两组中医证候积分、炎症因子(IL-6、CRP、TNF-α)、肾功能指标(SCr、BUN、Cys-C)水平均明显降低,且中医组低于西医组(P＜0.01).结论:养阴清热汤加减能够减轻阴虚性老年糖尿病肾病患者的炎症反应,缓解糖尿病肾病患者症状,有效改善患者的肾功能,治疗疗效显著.
Abstract
Objective To study the efficacy of modified Yangyin Qingre decoction in treating yin deficiency type of elderly dia-betic nephropathy and the influence on inflammatory factors of C-reactive protein(CRP)and interleukin-6(IL-6).Methods 65elderly patients with yin deficiency type of diabetic nephropathy admitted to hospital from January 2020to December 2022were divided into traditional Chinese medicine group(n=33)and western medicine group(n=32)according to the random number table method.Patients in the western medicine group received conventional western medicine symptomatic treatment,and patients in the traditional Chinese medicine group were given modified Yangyin Qingre decoction on the basis of western medicine group.The therapeutic effect,traditional Chinese medicine syndromes scores,inflammatory factors[CRP,IL-6,tumor necrosis factor-α(TNF-α)]and renal function indicators[serum creatinine(SCr),blood urea nitrogen(BUN),cystatin C(Cys-C)]were compared between both groups.Results The overall effective rate of treatment in the two groups was similar(P＞0.05),but the number of markedly effective cases in traditional Chinese medicine group was more than that in western medicine group(P＜0.05).After treatment,the traditional Chinese medicine syndromes scores,inflammatory factors(IL-6,CRP,TNF-α)and re-nal function indicators(SCr,BUN,Cys-C)were significantly decreased in both groups,and the scores and levels were lower in traditional Chinese medicine group than those in western group(P＜0.01).Conclusion Modified Yangyin Qingre decoction can re-duce the inflammatory response of elderly patients with yin deficiency type of diabetic nephropathy,relieve the symptoms,and ef-fectively improve the renal function of patients,with significant therapeutic efficacy.
