补阳还五汤加味联合依帕司他对气虚血瘀型糖尿病周围神经病变疗效及炎症因子、NRG1、Irisin的影响分析

摘要
目的:分析补阳还五汤加味联合依帕司他对气虚血瘀型糖尿病周围神经病变疗效及炎症因子、神经调节蛋白1(NRG1)、鸢尾素(Irisin)的影响.方法:选取2021年5月～2022年12月期间我院门诊诊治的100例气虚血瘀型糖尿病周围神经病变患者,以抛硬币法随机分为对照组(依帕司他治疗)与观察组(补阳还五汤加味联合依帕司他治疗),各有50例,观察气虚血瘀症状、血清指标、神经功能以及治疗效果.结果:治疗后,观察组气虚血瘀各症状评分、白细胞介素-6(IL-6)、IL-8、C反应蛋白(CRP)水平、NRG1低于对照组,Irisin高于对照组,均具有显著差异(P＜0.05);观察组正中神经、腓总神经的运动神经传导速度(MNCV)和感觉神经传导速度(SNCV)改善情况优于观察组(P＜0.05);观察组临床疗效总有效率较对照组高且差异显著(P＜0.05).结论:补阳还五汤加味联合依帕司他对气虚血瘀型糖尿病周围神经病变患者疗效显著,可改善患者气虚血瘀症状、NRG1、Irisin水平与神经传导速度,降低炎症因子水平.
Abstract
Objective:To analyze the therapeutic effect of modified Buyang Huanwu Decoction combined with epalrestat on diabetic peripheral neuropathy of qi deficiency and blood stasis type,and the influence on inflammatory factors,neuregulin 1(NRG1)and irisin.Methods:A total of 100patients with diabetic peripheral neuropathy of qi deficiency and blood stasis type who were diagnosed and treated in the hospital from May 2021to December 2022were selected,and randomly divided into the con-trol group(50patients treated with epalrestat)and the observation group(50patients treated with modified Buyang Huanwu De-coction combined with epalrestat).Results:After treatment,symptom scores and the levels of interleukin-6(IL-6),IL-8,C-reactive protein(CRP)and NRG 1 in the observation group were lower than those in the control group,and the level of irisin was higher than that in the control group(P＜0.05).The improvement of motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of median nerve and common peroneal nerve in the observation group was better than that in the con-trol group(P＜0.05).The total clinical effective rate in the observation group was higher than that in the control group(P＜0.05).Conclusion:Modified Buyang Huanwu Decoction combined with epalrestat is markedly effective in the treatment patients with diabetic peripheral neuropathy of qi deficiency and blood stasis type.The combined treatment can improve the symptoms of qi deficiency and blood stasis,NRG1and irisin levels and nerve conduction velocity,and lower the levels of inflammatory factors.
