温阳益气利水活血方剂联合沙库巴曲缬沙坦对慢性心力衰竭的疗效及对CD40L、Hcy水平的影响

摘要
目的:探究温阳益气利水活血方剂联合沙库巴曲缬沙坦对慢性心力衰竭患者血清可溶性CD40配体(CD40L)、同型半胱氨酸(Hey)水平的影响.方法:随机选取2020年6月～2022年6月我院收治的慢性心力衰竭患者100例,按照完全随机法分为对照组、联合组各50例.对照组采用沙库巴曲缬沙坦治疗,联合组在采用沙库巴曲缬沙坦联合温阳益气利水活血方剂治疗.评价两组患者心功能、CD40L、Hey、脑钠肽(BNP)水平、生活质量、临床症状严重程度、中医证候变化及临床疗效.结果:联合组LVRDD、LVPWD、IVS水平低于对照组,LVEF水平高于对照组(P＜0.05).联合组CD40L、Hey、BNP水平低于对照组(P＜0.05).联合组心衰评分、生活质量评分低于对照组(P＜0.05).联合组中医证候评分低于对照组(P＜0.05).联合组治疗总有效率高于对照组(P＜0.05).结论:温阳益气利水活血方剂联合沙库巴曲缬沙坦可显著改善患者临床症状,改善CD40L、Hey异常表达,促进心功能恢复,效果显著.
Abstract
Objective:To explore the effect of Wenyang Yiqi Lishui Huoxue Formula Combined with Sa Ku Ba Qu Xie Sha Tan on serum soluble CD40L(CD40L)and homocysteine(Hcy)levels in patients with chronic heart failure.Methods:100patients with chronic heart failure admitted to our hospital from June 2020to June 2022were randomly selected and divided into control group and combined group with 50cases each according to the complete randomized method.The control group was treated with Sa Ku Ba Qu Xie Sha Tan,and the combined group was treated with Sa Ku Ba Qu Xie Sha Tan and Wenyang Yiqi Lishui Huoxue Formula.The heart function,CD40L,Hey,BNP level,quality of life,severity of clinical symptoms,TCM syndrome changes and clinical efficacy were evaluated.Results:The level of lvrdd,lvpwd and IVS in the combined group was lower than that in the control group,and the level of LVEF was higher than that in the control group(P＜0.05).The levels of CD40L,Hcy and BNP in the combined group were lower than those in the control group(P＜0.05).The scores of heart failure and quality of life in the combined group were lower than those in the control group(P＜0.05).The TCM syndrome score of the combined group was lower than that of the control group(P＜0.05).The total effective rate of the combined group was higher than that of the con-trol group(P＜0.05).Conclusion:The combination of Wenyang Yiqi Lishui Huoxue Recipe and Sa Ku Ba Qu Xie Sha Tan can significantly improve the clinical symptoms,improve the abnormal expression of CD40L and Hcy,and promote the recovery of heart function.
