鼻内针刺鼻丘穴、内迎香穴联合中药贴敷治疗过敏性鼻炎疗效及对鼻腔分泌物EOS％、血清总IgE、IL-4及SP水平的影响

摘要
目的:探讨鼻内针刺鼻丘穴、内迎香穴联合中药贴敷治疗过敏性鼻炎疗效及对鼻腔分泌物嗜酸性粒细胞百分比(EOS％)、血清总免疫球蛋白E(IgE)、白细胞介素-4(IL-4)及P物质(SP)水平的影响.方法:选取2021年1月～2023年5月在本院门诊接受治疗的过敏性鼻炎患者60例,按治疗方法分为对照组和观察组,每组30例,对照组给予现代治疗方案(温生理盐水鼻腔+氯雷他定片口服+糠酸莫米松气雾剂喷鼻内),观察组基于以上治疗方案再行鼻内针刺联合穴位敷贴治疗,评估两组中医证候疗效,比较两组治疗前及治疗4周后的中医鼻部主要症状积分、鼻腔分泌物EOS％、嗜酸细胞活化趋化因子(eotaxin)含量和血清总IgE、IL-4及SP水平变化.结果:治疗4周后,观察组的中医症候疗效显著优于对照组(P＜0.05),总有效率显著高于对照组(P＜0.05);治疗4周后,两组喷嚏频发、清水样鼻涕、鼻痒、鼻塞共4项中医鼻部主要症状积分相较于治疗前有显著性降低(P＜0.05),两组鼻腔分泌物EOS％、eotaxin含量和血清总IgE、IL-4及SP水平相较于治疗前有显著性降低(P＜0.05),以上指标观察组均显著低于对照组(P＜0.05).结论:探讨鼻内针刺鼻丘穴、内迎香穴联合中药贴敷治疗过敏性鼻的中医证候疗效显著,能减轻鼻部症状,抑制鼻腔分泌物EOS％、eotaxin含量和血清总IgE、IL-4及SP水平.
Abstract
Objective To explore the efficacy of intranasal acupuncture at Biqiu acupoint and Neiyingxiang acupoint combined with traditional Chinese medicine application in the treatment of allergic rhinitis and its influence on nasal secretion eosinophil percentage(EOS％),serum total immunoglobulin E(IgE),interleukin-4(IL-4)and substance P(SP)levels.Methods 60patients with allergic rhinitis who received treatment at the outpatient department of our hospital from January 2021to May 2023were selected and divided into a control group and an observation group according to the treatment method.Each group had 30patients.The control group was given a modern treatment plan(warm physiological saline nasal cavity,oral loratadine tablets,and nasal spray of mometasone furoate aerosol),and the observation group received nasal acupuncture combined with acupoint ap-plication based on the above treatment plan to evaluate the efficacy of traditional Chinese medicine symptoms in both groups,Compare the changes in traditional Chinese medicine nasal symptom scores,EOS％of nasal secretions,eosinophil activated che-mokine(eotaxin)levels,and serum total IgE,IL-4,and SP levels between the two groups before and after 4weeks of treatment.Results The efficacy of TCM syndrome in the observation group after 4weeks of treatment was significantly better than that in con-trol group(P＜0.05),and the total effective rate was significantly higher(P＜0.05).After 4weeks of treatment,the scores of four main TCM nasal symptoms such as frequent sneezing,clear water-like nasal discharge,nasal itching and nasal congestion were obviously declined compared with those before treatment(P＜0.05),and the nasal secretion EOS％,eotaxin content and se-rum total IgE,IL-4and SP levels were significantly reduced compared to before treatment(P＜0.05),and the above indicators were significantly lower in observation group than those in control group(P＜0.05).Conclusion Intranasal acupuncture at Biqiu acupoint and Neiyingxiang acupoint combined with traditional Chinese medicine application has a significant efficacy of TCM syn-dromes in treating allergic rhinitis,and it can relieve the nasal symptoms,inhibit the nasal secretion EOS％and eotaxin content and serum total IgE,IL-4and SP levels.
