基于"厥阴为枢"论治化疗相关性腹泻初探

摘要
化疗相关性腹泻(chemotherapy-induced diarrhea,CID)是化疗常见的不良反应之一,引起发热、便血、电解质紊乱、肾损伤等变化,甚则危及生命.厥阴为阴阳出入,气机升降转枢之地,易发生阴阳之气不相顺接,出现易寒易热、易虚易实、两极转化错综复杂的病理变化.本文基于"厥阴为枢"理论,通过辨析《伤寒论》中下利的相关条文,认为厥阴下利与CID病因病机契合,CID病因主要为病后厥阴态、化疗寒热大毒;病机关键为厥阴枢机不利,阴阳失和,贼风内生,转归取决于阳气恢复程度.临证对寒热相兼,虚实错杂的下利,以和解枢机为治疗大法,和其寒热、调其虚实、重塑枢机,使补益不留邪、清热不伤正;对热利、寒利临证应以清肝经郁热,暖肝回厥止泻.
Abstract
Chemotherapy-induced diarrhea(CID)is one of the common complications after chemotherapy.Jueyin is a place where Yin and Yang enter and exit and the qi machine rises and turns.It is prone to the disjunction of Yin and Yang,and the complicated pathological changes such as cold and heat,deficiency and solid polarity transformation.Based on the theory of"Ju-eyin as the key point",this paper analyzed the relevant provisions and syndroms of the hypoxia of Jueyin in the Theory of Febrile Cold,and concluded that the pathogenesis of the hypoxia of Jueyin was consistent with the pathogenesis of CID.The main etiology of CID was the post-mortem Jueyin state and the severe toxicity of cold and heat during chemotherapy.The key of pathogenesis is the unfavorable Jueyin cardinal,the disharmony of Yin and Yang,the endogenous skua wind,the pathogenesis depends on the degree of Yang qi recovery.Clinical syndrome of cold and heat,deficiency and reality mixed under the benefit,to reconcile the cardinal for treatment,and its cold and heat,adjust its deficiency and reality,reshaping the cardinal,so that no evil,clear heat does not hurt positive;In the treatment of heat and cold,we should clear the liver meridian to suppress heat and warm the liver back to prevent diarrhea.
