摘要

目的:研究理中汤加味结合针刺对复发性口腔溃疡免疫功能及IL-17、Smad7、TGF-β1 的影响.方法:分析我院口腔科于 2022 年5 月~2023 年6 月接受治疗的106 例复发性口腔溃疡患者,依据治疗方式的差异将106 例患者分为观察组(n=54)和对照组(n=52),其中对照组在常规治疗的基础上给予针刺治疗,观察组在对照组的基础上结合理中汤加味治疗.对比观察两组患者的治疗效果和不良反应发生情况,临床症状(进食改善时间、溃疡愈合时间和溃疡疼痛消失时间),并且比较两组患者治疗前后的免疫功能指标(CD3+、CD4+、CD4+/CD8+),IL-17、Smad7、TGF-β1 水平.结果:观察组的治疗总效率显著高于对照组(P<0.05).经过治疗后,观察组在进食改善时间、溃疡愈合时间和溃疡疼痛消失时间上均明显低于对照组(P<0.05).与治疗前相比,两组患者治疗后的CD3+、CD4+、CD4+/CD8+水平均有明显上升,且观察组的各项免疫功能指标显著优于对照组(P<0.05).治疗后与治疗前相比,两组患者IL-17、Smad7、TGF-β1 水平均有明显改善,且观察组优于对照组,差异具有统计学意义(P<0.05).两组患者的不良反应相比差异无统计学意义(P>0.05).结论:理中汤加味结合针刺可提高复发性口腔溃疡的治疗效果,有效改善临床症状,可能是与下调IL-17、TGF-β1 水平、上调Smad7 的表达和增强免疫功能有关.

Abstract

Objective:To study the effects of Lizhong decoction combined with acupuncture on immune function,IL-17,Smad7and TGF-β1in patients with recurrent oral ulcer.Methods:Analysis was made on 106patients with recurrent aphthous ulcer who received treatment in the outpatient department of Stomatology of our hospital from May 2022to June 2023.According to the differences in treatment methods,106patients were divided into observation group(n=54)and control group(n=52),in which the control group was given acupuncture treatment on the basis of conventional treatment.Observation group was combined with Lizhong decoction flavor treatment on the basis of control group.The therapeutic effect and occurrence of adverse reactions,clinical symptoms(time to improve eating,time to heal ulcer and time to disappear ulcer pain)of the two groups were compared,and the immune function indicators(CD3+,CD4+,CD4+/CD8+),IL-17,Smad7and TGF-β1levels of the two groups were compared before and after treatment.Re-sults:The total treatment efficiency of observation group was significantly higher than that of control group(P<0.05).After treat-ment,the improvement time of eating,ulcer healing time and ulcer pain disappearance time in the observation group were significantly lower than those in the control group(P<0.05).Compared with before treatment,the levels of CD3+,CD4+and CD4+/CD8+in both groups were significantly increased after treatment,and the immune function indexes in the observation group were significantly better than those in the control group(P<0.05).After treatment,the levels of IL-17,Smad7and TGF-β1in both groups were significantly improved,and the observation group was better than the control group,the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups(P<0.05).Conclusion:Lizhong decoction combined with acupuncture can improve the therapeutic effect of recurrent oral ulcer and improve clinical symptoms effectively,which may be related to down-regulating the levels of IL-17and TGF-β1,up-regulating the expression of Smad7and enhancing immune function.
