摘要
目的 分析江苏省女性居民1990-2019年间卵巢癌的疾病负担变化趋势及相关危险因素的归因情况.方法 利用2019年全球疾病负担中的江苏省卵巢癌数据,分析1990-2019年卵巢癌的发病率、死亡率、伤残调整寿命年(DALY)、早死寿命损失年(YLL)、伤残寿命损失年(YLD)等指标,采用线性回归和联结点回归法,应用Joinpoint 4.6.0.0软件计算平均年度变化百分比(AAPC),利用人群归因分数(PAF)估计高空腹血糖(FPG)、高体质指数(BMI)和职业性石棉暴露危险因素的归因疾病负担水平.结果 估算1990年江苏省女性卵巢癌的新发病例、死亡病例和DALY分别为804例、540例和17 158.83人年,2019年分别为3 154例、1 966例和54 921.17人年.30年间标化发病率、标化死亡率和标化DALY率分别年均增长2.16%、1.39%和1.35%(t=21.900~54.033,P<0.001),其中DALY构成95%以上为YLL.2019年江苏省女性卵巢癌的发病、死亡高峰约在70岁,均较1990年明显前移.高FPG和高BMI的PAF呈逐年上升趋势(t=12.338~71.323,P<0.001).结论 江苏省女性卵巢癌的疾病负担持续加重.应积极开展卵巢癌相关的健康宣教,重视控制血糖、减重等健康生活方式在卵巢癌防治中的积极作用.
Abstract
Objective To analyze the disease burden of ovarian cancer and its related risk factors among female residents in Jiangsu province from 1990 to 2019.Methods Based on the data from the Global Burden of Disease(GBD)Study 2019 on ovarian cancer in Jiangsu province,we evaluated the rates of incidence,mortality,disability-adjusted life year(DALY),years of life lost(YLL),and years lived with disability(YLD)of female residents.The average annual percentage change(AAPC)was calculated using Joinpoint 4.6.0.0 software.The population attributable fraction(PAF)was used to estimate the attributable burden of high fasting glucose(FPG),high body mass index(BMI),and occupational asbestos exposure.Results In 1990,the numbers of newly cases,deaths,and DALYs were 804,540 and 17 158.83 due to female ovarian cancer in Jiangsu province.In 2019,they were 3 154,1 966,and 54 921.17,respectively.In the past 30 years,the standardized rates of incidence,mortality,and DALY increased annually by 2.16%,1.39%,and 1.35%,respectively(t=21.900-54.033,P<0.001).More than 95%of DALYs were attributable to YLL.The peak incidence and mortality occurred around group aged 70 years old in 2019,both of which significantly moved forward compared to 1990.The PAF values for high FPG and high BMI showed annually increased trends(t=12.338-71.323,P<0.001).Conclusions The burden of female ovarian cancer in Jiangsu province continued to rise.Effective health education on ovarian cancer and healthy lifestyles,including glycemic control and weight management,should be paid attention.
