北京市人群疾病负担研究中的伤残权重调查与分析

目的 估算2018年北京市人群的伤残权重,为准确评估人群疾病负担提供可参考的数据资料.方法 以2013年全球疾病负担(global burden of disease,GBD)研究中估算全球人群伤残权重的问卷为基础,结合北京市人群实际情况,对231种健康状态展开问卷调查.将纳入的健康状态进行两两配对,建立Probit模型对伤残权重进行估算.结果 北京地区人群伤残权重值最高的健康状态为颈部脊髓损伤(未治疗),其权重值为0.992,其后第2~5位依次为颈部以下的脊髓损伤(未治疗)、颈部以下的脊髓损伤(接受治疗)、颈部脊髓损伤(接受治疗)、中风(长期后果、重度加认知问题).权重值最低的状态为老花眼,其权重值为0.081.本研究获得的伤残权重值整体高于GBD研究,与韩国人群结果更为接近.北京市人群的权重值与GBD结果相差最大的健康状态主要集中在终末期疾病.结论 北京市人群对健康不良影响的严重程度认知与GBD人群存在较大的文化差异,与韩国人群结果较为接近,本研究产生的健康状态权重值比GBD的结果更具有参考性,对于准确评价北京市人群疾病负担提供了可靠的参考依据.
Abstract
Objective To estimate the disability weights of the population in Beijing in 2018 and provide reference data for the accurate assessment of the population disease burden.Methods Based on the questionnaire used by The Global Burden of Disease(GBD)Study 2013,and combined with the characteristics of population in Beijing,a modified questionnaire survey was conducted based on 231 kinds of health status.The included health status was pair-to-pair matched,and the Probit model was established to estimate the disability weight.Results The health condition with the highest disability weight in Beijing was cervical spinal cord injury(untreated),and its disability weight was 0.992.The next two to five were spinal cord injury below the neck(untreated),spinal cord injury below the neck(treated),spinal cord lesion at neck level(treated),stroke(long-term consequences,severe plus cognitive problems).The state with the lowest disability weight is presbyopia(0.081).The result in our study is higher than that in the GBD study,which is more similar to the result of the Korean population.The health status with the biggest difference between the disability weight and GBD result in Beijing population mainly concentrated in end-stage diseases.Conclusions There is a big cultural difference between the cognition of the severity of adverse health effects of the Beijing population and that of the GBD population,and the result is close to that of the Asian population.The disability weight in our study are more referable than the result from GBD,and provides a reliable reference for accurately evaluating the disease burden of Beijing population.
