达格列净联合格列齐特治疗血糖控制欠佳肥胖2型糖尿病的效果

摘要
目的 探讨达格列净联合格列齐特强化降糖治疗血糖控制欠佳肥胖2型糖尿病的效果观察.方法 选取天津市河西区梅江街社区卫生服务中心在2022年2月-2023年4月收治的80例血糖控制欠佳肥胖2型糖尿病患者的病历资料进行分析,随机分为对照组(n=40)和实验组(n=40).对照组采用格列齐特治疗,实验组在对照组的基础上联合达格列净治疗.分析比较两组临床疗效、血糖水平、游离脂肪酸(FFA)、过氧化物酶体增殖物激活受体γ(PPARγ)、胰岛素抵抗指数(HOMA-IR)水平、不良反应发生率.结果 两组相比,实验组疗效更高(P＜0.05);用药前,两组FPG、2h PG、HbA1c水平相比,差别不大(P＞0.05);用药后,两组均降低且实验组较对照组低(P＜0.05);用药前,两组FFA、PPARγ、HOMA-IR水平相比,差别不大(P＞0.05);用药后,两组FFA水平均降低,两组PPARγ水平均升高,且实验组变化幅度比对照组更明显(P＜0.05);用药后,两组HOMA-IR水平均降低,两组相比,实验组变化幅度高于对照组(P＜0.05);两组不良反应发生率比较,差异不大(P＞0.05).结论 达格列净联合格列齐特治疗血糖控制欠佳肥胖2型糖尿病,疗效显著,可有效控制患者血糖水平,并提高胰岛素敏感度.
Abstract
Objective To investigate the effect of dagglinide combined with gliclazide in the treatment of obesity type 2 diabetes with poor glycemic control.Methods The data of 80 obese type 2 diabetes patients with poor blood glucose control who were admitted to the community health service center in Meijiang Street,Hexi District,Tianjin from February 2022 to April 2023 were analyzed.They were randomly divided into the control group(n=40)and the experimental group(n=40).The control group received treatment with gliclazide,while the experimental group received combined treatment with dapagliflozin on the basis of the control group.Analyze and compare the clinical efficacy,blood glucose levels,free fatty acids(FFA),and peroxisome proliferator activated receptors γ(PPARγ),Homeostatic Model Assessment of Insulin Resistance(HOMA-IR)levels and incidence of adverse reactions between two groups.Results Compared with the two groups,the experimental group had a higher therapeutic effect(P＜0.05);before medication,there was no significant difference in the levels of FPG,2-hour PG,and HbA1c between the two groups(P＞0.05);after medication,both groups showed a decrease and the experimental group was lower than the control group(P＜0.05);before medication,both groups of FFA and PPARγ、the difference in HOMA-IR levels is not significant(P＞0.05);after medication,both groups had a decrease in FFA levels,and both groups had PPARγ the levels all increased,and the change in the experimental group was more significant than that in the control group(P＜0.05);after medication,the HOMA-IR levels in both groups decreased,and compared to the control group,the change in the experimental group was higher(P＜0.05);the incidence of adverse reactions between the two groups was not significantly different(P＞0.05).Conclusion Daggligin combined with gliclazide is effective in the treatment of obesity type 2 diabetes with poor blood glucose control,which can effectively control the blood glucose level of patients and improve insulin sensitivity.
