Analysis of the current situation and influencing factors of surrogate decision-makers participation in clinical decision-making in emergency stroke patients
Objective To explore the current situation and related influencing factors of surrogate decision-makers'participation in clinical decision making in emergency stroke patients,and to provide reference for improving their ability to participate in medical decision making.Methods From April 2021 to April 2024,210 cases of emergency stroke patients in Tianjin Beichen Hospital were selected as study subjects by convenient sampling method.General data questionnaire,Simple Coping Style Questionnaire(SCSQ)and decision Tendency Scale(CPS)were used to investigate.Multiple linear regression analysis was used to explore the influencing factors of the current situation of participation in clinical decision-making expectations.Results The total score of the decision preference scale for emergency stroke patients was(48.02±2.33),and the dimension of decision need was the highest.Single-factor results showed that the total CPS scores of emergency stroke patients substituted for decision makers were significantly different among education level,stroke cognition,medical in surance type,treatment attitude and care burden level(P<0.05).There was a significant positive correlation between the expectation of alternative decision makers to participate in clinical decision-making and the positive coping style,and a significant negative correlation between the expectation and the negative coping style(P<0.01).The results of multiple linear regression analysis showed that the expectation of emergency stroke patients to participate in clinical decision making was affected by education level,stroke cognition level,treatment attitude and care burden level.Conclusion The expectation of emergency stroke patients to participate in clinical decision making as an alternative decision maker is at a medium or high level.High education level,deep stroke cognition,positive treatment attitude and heavy care burden are the main influencing factors of high expectation of emergency stroke patients to participate in clinical decision-making instead of decision makers.
