The application value of MRI imaging features in the diagnosis of knee joint sports bone contusion lesion location and size
Objective To analyze the value of MRI in the clinical diagnosis of exercise-induced bone contusion of knee joint. Methods 160 patients with exercise-induced bone contusion of knee joint admitted to our hospital were selected as the research objects. All patients were required to undergo CT and MRI examination. The sensitivity, specificity and accuracy of CT and MRI examination items were compared with the results of arthroscopy as the gold standard. Results MRI examination of patients with knee joint exercise bone contusion in this study showed that 186 lesions were found in patients with knee joint exercise bone contusion in this study,of which 84 cases(52.50%)were single cases and 76 cases(47.50%)were multiple cases. In the multiple cases, the main affected site was the lateral femoral malleolar 94,64 femoral medial malleolus,Patella 2,Tibia medial malleolar 6,8 lateral malleolar tibia, the proportion of 12 tibial plateau was 50.54%,34.41%,1.25%,3.75%,5.00%and 7.50%,respectively.Symptoms of periarticular tissue injury:there were 26 ligament injuries,8 meniscus injuries,8 lateral collateral ligaments injuries,8 medial collateral ligaments injuries,2 patellar ligaments injuries,4 posterior cruciate ligaments injuries,4 anterior cruciate ligaments injuries,2 lateral meniscus injuries, and 4 cartilage injuries. They accounted for 16.25%,5.00%,5.00%,5.00%,1.25%,2.50%,2.50%,1.25%,2.50%,respectively.The disease types of the patients in this study were:70 patients with type I,22 patients with type II,and 68 patients with type III.Type I:There are map-like or patchy abnormal signals in the cancellous bone near metaphyseal or epiphysis of knee joint with blurred boundaries, indicating high signal at STER,complex signal at T2WI,and low signal at T1WI.MRI findings of type II patients:In addition to abnormal findings, some patients also had discontinuity of the bone cortex. The MRI findings of type III patients showed abnormal signals in local lesions near the subcortex. After comparison,it was found that the sensitivity, specificity, accuracy and consistency of MRI items were higher than those of CT(P<0.05).Conclusion MRI has a better clinical effect in the diagnosis of exercise-induced bone contusion of knee joint, which is helpful for clinical localization of the lesions and evaluation of the scope and size of the lesions, and is worthy of clinical application and promotion.
