Survey on the knowledge of atrial fibrillation and anticoagulation among community general practitioners in Minhang District
Objective To analyze the knowledge of atrial fibrillation and anticoagulation among community general practitioners in Minhang District,Shanghai,and to provide powerful ideas for subsequent physician training.Methods A questionnaire survey was conducted among 504 general practitioners in 13 community health service centers in Minhang District,Shanghai from January 2021 to March 2022 using cluster sampling method.Results 100.00%of the doctors had received strong training on AF and anticoagulation related knowledge,62.70%of the doctors believed that it was necessary to use three kinds of scores to evaluate patients before anticoagulation treatment,and 65.67%of the doctors believed that it was very necessary to carry out AF related knowledge health education for AF patients.77.58%of the physicians believed that it was necessary to understand the patients'views on the risks and approval of anticoagulation therapy,and 79.76%of the physicians believed that it was necessary to follow up the medication compliance and quality of life of anticoagulation patients.100.00%of physicians believed that AF was the most common arrhythmia in clinical practice,87.50%of community general practitioners believed that AF was related to stroke,and only 26.39%of physicians believed that anticoagulant drugs should be the first choice for the treatment of AF patients.18.45%of the physicians were not clear about the contraindications of drugs in the treatment of medium-high risk AF patients with CHADS2≥2,17.06%of the physicians were not clear about the limitations of the use of anticoagulant warfarin,and 100.00%of the physicians could accurately tell the characteristics of ideal anticoagulant drugs.100.00%of the physicians fully mastered CHA2DS2VASc scoring system,86.51%of the physicians mastered the indications of NOACs,and 91.47%of the physicians could correctly say the dose adjustment plan to optimize VKA.100.00%of the physicians could correctly name the CHA2DS2-VASC score range for long-term anticoagulant therapy,and 88.69%of the physicians were familiar with the 2020 ESC guidelines for ventricular rate control.Conclusion In general,community general practitioners in my district have a positive attitude towards the prevention and treatment of AF anticoagulation,and have a good grasp of the relevant knowledge of AF and anticoagulation,but there are still a small number of physicians have the problem of ignorance and lack of knowledge.Follow-up physician training should be done well.
