Relationships of ultrasound elastic imaging parameters with pathological features and prognosis in patients with papillary thyroid carcinoma
Objective To investigate the relationships between ultrasound elastic imaging parameters,pathological features and prognosis in patients with papillary thyroid carcinoma(PTC).Methods A total of 121 patients with PTC(PTC group)and 45 patients with benign thyroid tumors(benign control group)were selected.Patients in both groups un-derwent ultrasound elastography examination.We calculated the elastic ratio and red area ratio,and analyzed the relation-ships between the two indicators and the pathological characteristics of PTC.PTC patients were followed up for 3 years and the 3-year survival rate was calculated.Log-rank test was used to compare prognosis and survival,and Cox regression mod-el was used to analyze the factors affecting the prognosis of PTC patients.Results The elastic ratio and red area ratio of the PTC group were higher than those of the benign control group(all P<0.05).The elastic ratio and red area ratio of PTC patients with TNM stage Ⅲ and Ⅳ and lymph node metastasis were higher than those with TNM stage Ⅰ and Ⅱ and with-out lymph node metastasis(all P<0.05).There were no statistically significant differences in elastic ratio or red area ratio among PTC patients with different age,sex,tumor location and tumor number(all P>0.05).During the follow-up period of 33-36 months,25 patients died.The 3-year survival rate was 79.33%(96/121).Taking the mean values of elastic ra-tio and red area ratio as the critical values,PTC patients were divided into the high elastic ratio group(57 cases)and low elastic ratio group(64 cases),the high red area ratio group(55 cases)and low elastic ratio group(66 cases).The 3-year survival rates of the high elastic ratio group and high red area ratio group were higher than those of the low elastic ratio group and low red area ratio group,respectively(all P<0.05).Cox regression model showed that TNM stage,lymph node metastasis,elastic ratio and red area ratio were independent risk factors affecting the prognosis of PTC patients(all P<0.05).Conclusion The red area ratio and elastic ratio of ultrasound elastic imaging parameters in PTC patients are high,and they are closely related to the clinical stage and lymph node metastasis in PTC patients.
