目的 探讨重复经颅磁刺激(rTMS)联合高压氧(HBO)治疗对颅脑损伤患者认知功能、脑血流及不良情绪的影响.方法 将 80 例颅脑损伤患者随机分为实验组和对照组,每组 40 例.对照组接受常规治疗,实验组在常规治疗的基础上接受rTMS和HBO治疗,为期 4 周.治疗前后评估患者的认知功能(MoCA评分和P300)、脑血流情况(TCD)和情绪状态(HAMA和HAMD评分).结果 治疗后,两组患者的各项指标均有显著改善(P<0.05).与对照组相比,实验组在认知功能改善(MoCA评分增加、P300 潜伏期缩短、P300 波幅增加)、脑血流改善(Vm、Vd、Vs增加,PI降低)和情绪状态改善(HAMA和HAMD评分降低)方面均显著优于对照组(P<0.05).结论 rTMS联合HBO治疗能显著改善颅脑损伤患者的认知功能、脑血流状况和情绪状态,优于单纯常规治疗方法,从而为颅脑损伤患者的康复提供了新的治疗选择.
The Effect of Repetitive Transcranial Magnetic Stimulation Combined with Hyperbaric Oxygen on Cognitive Function,Cerebral Blood Flow,and Negative Emotions in Patients with Traumatic Brain Injury
Objective To explore the effect of repetitive transcranial magnetic stimulation(rTMS)combined with hyperbaric oxygen(HBO)therapy on cognitive function,cerebral blood flow,and negative emotions in patients with traumatic brain injury(TBI).Methods A total of 80 patients with TBI were randomly divided into an experimental group and a control group,with 40 patients in each group.The control group received routine treatment,while the experimental group received rTMS and HBO therapy in addition to routine treatment for a duration of four weeks.Cognitive function(MoCA scores and P300),cerebral blood flow(TCD),and emotional state(HAMA and HAMD scores)were assessed before and after treatment.Results After treatment,all indicators in both groups showed significant improvement(P<0.05).Compared to the control group,the experimental group demonstrated significantly better improvement in cognitive function(increased MoCA scores,shortened P300 latency,and increased P300 amplitude),cerebral blood flow(increased Vm,Vd,and Vs,decreased PI),and emotional state(lower HAMA and HAMD scores)(P<0.05).Conclusions rTMS combined with HBO therapy significantly improves cognitive function,cerebral blood flow,and emotional state in patients with traumatic brain injury,outperforming routine treatment alone.This provides a new treatment option for the rehabilitation of TBI patients.
