This paper summarizes the important significance,opportunities faced and challenges of elderly health management,and exploration of development of the health management mode of"six cares"for the elderly in East China Sanatorium,and elaborates on the main contents of the services,the effectiveness of the practice,and the outlook of the future for"six cares",so as to provide the ideas for the construction of the medical and elderly care service system in the context of integration of medical and elderly care integration.
