Efficacy of topical versus intravenous tranexamic acid in total hip arthroplasty
Objective:To explore the efficacy of topical and intravenous tranexamic acid in total hip arthroplasty,and clarify the difference between them.Methods:Forty cases of patients who underwent total hip arthroplasty were retrospectively analyzed and divided into a tranexamic acid administration group(topical group)and an intravenous drip of tranexamic acid group(intravenous group)according to their administration method with 20 cases each.The topical injection of tranexamic acid in the joint cavity was adopted in the topical group while the intravenous drip of tranexamic acid was used in the intravenous group.Differences in hemoglobin(Hb)level and its amount of change,transfusion rate,hospital stay and the incidence of venous thromboembolism(VTE)were compared between the two groups.Results:There was no statistical significance in the differences of intraoperative Hb level,blood transfusion rate,average hospital stay and VTE incidence between the two groups(P>0.05).The Hb level on the first postoperative day was higher,and the amount and rate of decrease of Hb level in the postoperative period were lower in the topical group than the intravenous group(P<0.05).Conclusion:In total hip arthroplasty,intra-articular topical tranexamic acid was more effective in improving postoperative bleeding compared with intravenous drip of tranexamic acid,and neither mode of administration increases the risk of VTE.
