安徽省青阳县阴道镜检查女性HPV感染情况与病理分型分析

摘要
目的:了解阴道镜检者HPV感染亚型分布等情况,为宫颈病变的精准预防、感染管理和控制提供参考.方法:选取 577 名行阴道镜检查者为研究对象,通过问卷采集患者基础信息,并进行HPV、TCT检查.结果:12.1%女性有阳性体征,51.1%为TCT检测阳性.HPV检测阳性率为 72.8%,多重感染率为 27.6%,高危型感染率为 86.9%.HPV感染亚型以高危型为主,且在已婚、存在阳性体征、TCT检测和病理检查均阳性的妇女中多见.同时,年龄＞55 岁、已绝经、有阳性体征和HPV高危亚型感染者中病理结果为CIN2/C1N3/CA的比例升高.结论:青阳县女性HPV感染率高,以HPV52、HPV16、HPV58、HPV53 和HPV51 高危亚型为主,年龄是影响HPV感染、严重程度和愈后的重要因素,今后须定期开展筛查,对症处理和随访,避免持续感染和多重感染,降低宫颈癌的发病率和死亡率.
Abstract
Objective:To understand the distribution of HPV infection subtypes in women undergoing colposcopy so as to provide reference for the precise prevention,infection management and control of cervical lesions.Methods:Five hundred and seventy-seven patients were selected as the research subjects and their basic information was collected by questionnaires,and HPV and TCT examinations were performed for all of them.Results:The women with positive signs and with positive TCT results accounted for 12.1%and 51.1%.The positive rate of HPV detection,the rates of multiple infection and high-risk infection accounted for 72.8%,27.6%and 86.2%,respectively.The subtypes of HPV infection were predominantly high-risk,which was more common in married women with positive signs and positives in TCT testing and pathological examinations.At the same time,the women over 55 years old,menopausal,with positive bodies and high-risk subtypes of HPV infection had an increased proportion of CIN2/C1N3/CA pathological results.Conclusion:The rate of HPV infection in women of Qingyang County is high,and its manifestations are mainly the high-risk subtypes of HPV52,HPV16,HPV58,HPV53 and HPV51.Age is an important factor affecting HPV infection,severity and outcome.In the future,regular screening,symptomatic treatment and follow-up should be carried out to avoid persistent infection and multiple infections and reduce the incidence and mortality of cervical cancer.
