社区全科医生综合管理签约2型糖尿病患者的效果评价
Evaluation of the effect of comprehensive management of type 2 diabetes patients by contracted general practitioners in the community

摘要
目的:观察 2 型糖尿病患者在社区全科签约医生的综合管理干预后血糖、血脂等指标变化,评价 2 型糖尿病患者综合目标控制情况,为社区开展糖尿病管理工作提供参考.方法:选取 2020 年 8 月-2022 年 12 月在杭州拱宸桥街道社区卫生服务中心门诊就诊,血糖控制未达标,且与全科医生签约的 80 例 2 型糖尿病患者为观察组,同时在社区慢病信息系统里选取 80 例未签约,且血糖控制未达标的 80 例 2 型糖尿病患者为对照组.观察组中男性 34 例,女性 46例,年龄 30～85 岁,平均年龄(63.12±2.14)岁;糖尿病病程 1～20 年;对照组男 36例,女 44例,年龄 32～86 岁,平均年龄(63.24±1.86)岁,糖尿病病程 2～20 年.观察管理前后两组患者血糖、血脂、血压、体重指数以及患者对糖尿病相关知识知晓率的变化,调查患者对全科医生的满意度.结果:观察组患者血糖、血脂、血压、体重指数等指标的控制均优于对照组,血糖控制达标率 90%,糖尿病相关知识知晓率及对全科医生的满意率均高于对照组,差异均有统计学意义(均P＜0.05).结论:社区全科医生对签约 2 型糖尿病患者综合管理能有效降低血糖、血脂、血压、体重指数等水平,提高糖尿病相关知识知晓率及对签约医生的满意率.
Abstract
Objective:To observe the changes of blood glucose,blood lipid and other indexes of type 2 diabetes patients after comprehensive management intervention by contracted general practitioners in the community,and evaluate the comprehensive goal control of type 2 diabetes patients,so as to provide reference for development of community diabetes management.Methods:Eighty patients with type 2 diabetes who failed to reach the standard in blood glucose control and contracted with the general practitioners of Gongchen Bridge Community Health Service Center from August 2020 to December 2022 were selected as the observation group,and at the same time,eighty unsigned type 2 diabetes patients with substandard blood glucose control were selected in the community chronic disease information system as the control group.In the observation group there were 34 males,and 46 females,the age was from 30 to 85 years and the average age was(63.12±2.14)years;the history of the disease was 1-20 years;in the control group there were 36 males and 44 females,the age was 32-86 years,and the average age was(63.24±1.86)years and the history of the disease was 2-20 years.The changes of blood glucose,blood lipid,blood pressure and body mass index of patients before and after management were observed,as well as the awareness rate of diabetes-related knowledge of patients and the satisfaction rate of general practitioners was statistically investigated between the two groups.Results:The blood glucose,blood lipid,blood pressure and body mass index of the patients were controlled better in the observation group than those in the control group,and the rate of reaching the standard of blood glucose control was 90%,the awareness rate of diabetes related knowledge and the satisfaction rate of the general practitioners in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Comprehensive intervention management of type 2 diabetes patients under the contract of community general practitioners can effectively reduce the levels of blood glucose,blood lipids,body mass index and other indicators,improve the awareness rate of diabetic related knowledge,and the satisfaction rate of contracted doctors.
