Evaluation of the effect of community health management group intervention on type 2 diabetes patients
Objective:To analyze the influence of community health management group on blood glucose,blood lipid and self-management ability of patients with type 2 diabetes.Methods:Sixty patients with type 2 diabetes in Dongshe Village and Maodian Village of Liantang Community were randomly selected as an intervention group and a control group.The control group was given routine guidance,and the intervention group was managed through the health management group mode,the blood glucose and lipid levels of the two groups after six months of intervention,and the improvement of self-management ability of patients with poor blood sugar control were compared.Results:After 6 months of intervention,the exceedice standard rates of blood glucose and blood lipid of the patients in the intervention group were significantly lower than that in the control group(P<0.05).The self-management ability of the patients with poor blood sugar control was higher than that in the control group(P<0.05).Conclusion:Community health management group intervention on patients with type 2 diabetes can effectively reduce their blood glucose and lipid levels,and improve their self-management ability.
