Effect of community weight management on the weight compliance supervision and readmission rate in the patients with chronic heart failure
Objective:To explore the effect of weight management under the guidance of family doctors'team on the weight compliance supervision and readmission rate in patients with chronic heart failure(CHF).Methods:Two hundred and forty-nine CHF patients treated in Yichuan Community Health Service Center of Putuo District,Shanghai from June 2019 to May 2020 were selected and divided into an intervention group with 128 cases and a control group with 128 cases by random number table method.The intervention group received the weight management guidance and intervention of the family doctors'team,while the control group received routine health education.Results:Before intervention,there were no significant differences in weight compliance supervision distribution and readmission rate(P>0.05).After 1 year management,the weight compliance supervision distribution and self-management scale score in the intervention group were significantly higher than those in the control group,and readmission rate was lower in the intervention group than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Weight intervention under the guidance of family doctors'team can help improve the patients'weight supervision compliance and reduce the readmission rate.
