摘要
目的:探讨临床药师在老年 2型糖尿病肾病伴甲状腺功能减退患者的药物治疗中发挥的作用和意义.方法:临床药师通过参与 1例老年 2型糖尿病肾病伴甲状腺功能减退患者的用药方案制定,对降糖用药、降压药物、甲减药物、利尿药物等提供个体化的用药指导.结果:临床药师及时发现并解决了药物治疗中的问题,提出了药学监护要点,并对患者进行用药教育.结论:临床药师可充分发挥其在老年慢性病患者药物治疗中的优势,协助临床提高药学服务质量.
Abstract
Objective:To explore the role and significance of clinical pharmacists in the drug treatment and pharmaceutical care of an elderly patient with type 2 diabetic nephropathy and hypothyroidism.Methods:Clinical pharmacists participated in the formulation of a medication plan for the patient and provided individualized medication guidance on hypoglycemic,antihypertensive,hypothyroidism and diuretic medication and so on.Results:Clinical pharmacists promptly discovered and solved problems during drug treatment,put forward the key points of pharmaceutical care,and provided medication education to patients.Conclusion:Clinical pharmacists can gives full play to their advantages in the drug treatment of elderly patients with chronic diseases and assist clinicians in improving the quality of pharmaceutical care to patients.
