杵针加牵引结合颈肩操治疗神经根型颈椎病的临床研究

摘要
目的:观察杵针加牵引结合颈肩操治疗神经根型颈椎病的临床效果.方法:收集 2020 年 1 月—2023 年12 月于上海市徐汇区华泾镇社区卫生服务中心针灸科门诊治疗的神经根型颈椎病患者 84 例,按照数字随机分成两组各42 例.观察组采取杵针加颈椎牵引结合颈肩操练习,对照组采取颈椎牵引结合颈肩操练习.两组均治疗两个疗程.分别采用视觉模拟量表(visual analogue scale,VAS)和Northwood Park颈痛量表(NPQ)评估颈肩疼痛程度与颈部功能,以及通过神经肌电图评估神经传导速度,并比较两组疗效.结果:两组治疗后VAS评分、NPQ评分以及神经传导速度均较治疗前有明显改善(均P＜0.05),且观察组改善程度显著高于对照组(均P＜0.05).观察组总疗效为92.88%(39/42),高于对照组的 76.19%(32/42,P＜0.05).结论:杵针加颈椎牵引结合颈肩操练习治疗神经根型颈椎病疗效佳,能更好地缓解疼痛,加快颈部功能、神经功能恢复.
Abstract
Objective:To observe the clinical effect of pestle needle combined with cervical traction and neck and shoulder exercise in the treatment of cervical spondylotic radiculopathy.Methods:Eighty-four cases of cervical spondylotic radiculopathy were collected in Outpatient Department of Acupuncture and Massage Huajing Community Health Service Center of Xuhui District,Shanghai from Jan.2020 to Dec.2023,and randomly divided into two groups with 42 cases each according to the numbers.The patients in the observation group were treated with pestle needle combined with cervical traction and neck and shoulder exercises,while those in the control group were treated with cervical traction and neck and shoulder exercises.Both groups were treated for two courses.Visual analogue scale(VAS),Northwood Park(NPQ)scores were used to evaluate the degree of neck and shoulder pain and neck function,and evaluate nerve conduction velocity through electromyography and the effects in both groups were compared.Results:The VAS,NPQ and nerve conduction velocity were improved obviously between the two groups before and after treatment(P＜0.05),and the improvement in the observation group was higher than that in the control group(P＜0.05).The total effective rate was 92.88%(39/42,P＜0.05)in the observation group,which was higher than that 76.19%(32/42,P＜0.05)in the control group.Conclusion:The pestle needle combined with cervical traction and neck and shoulder exercise has a definite effect in the treatment of cervical spondylotic radiculopathy,and can better alleviate pain,and accelerate the recovery of neck function and nerve function.
