中药熏蒸联合针刺治疗脑梗死偏瘫患者对脑血流动力学以及肢体功能的影响
Effect of Chinese Medicine Fumigation Combined with Acupuncture on Cerebral Hemodynamics and Limb Function in Cerebral Infarction Hemiplegia Patients

摘要
目的 探讨脑梗死偏瘫患者在常规康复治疗与针刺治疗基础上增加中药熏蒸治疗对脑血流动力学以及肢体功能的影响.方法 便利选取2019年1月—2023年2月常熟市第二人民医院收治的108例脑梗死偏瘫患者为研究对象,根据抽签法分为两组,对照组(54例)在常规康复治疗基础上增加针刺治疗,观察组(54例)在对照组治疗方案基础上接受中药熏蒸治疗.对比两组脑血流动力学指标水平、神经功能、肢体运动功能.结果 干预后,观察组大脑动脉的阻力指数、搏动指数指标水平低于对照组,差异有统计学意义(P均<0.05).观察组平均血流速度指标水平为(58.71±6.23)cm/s,高于对照组的(53.80±5.65)cm/s,差异有统计学意义(t=4.290,P<0.05).观察组美国国立卫生研究院卒中量表评分低于对照组,上肢功能、下肢功能评分高于对照组,差异有统计学意义(P均<0.05).结论 在常规康复治疗和针刺治疗的基础上增加中药熏蒸治疗,可促使脑梗死偏瘫患者脑血流动力学指标明显改善,大幅度减轻神经功能损伤程度,全面提高肢体运动功能.
Abstract
Objective To investigate the effects of adding traditional Chinese medicine fumigation on cere-bral hemodynamics and limb function of cerebral infarction hemiplegia patients on the basis of conventional re-habilitation therapy and acupuncture treatment.Methods 108 cases of hemiplegic patients with cerebral infarc-tion admitted to the Second People's Hospital of Changshu City from January 2019 to February 2023 were con-veniently selected as the study object,and were divided into two groups according to the lottery method,the control group(54 cases)added acupuncture treatment on the basis of conventional rehabilitation treatment,and the observation group(54 cases)received traditional Chinese medicine fumigation treatment on the basis of the treatment program of the control group.Compared the cerebral hemodynamic index level,neurological function,and limb motor function of the two groups.Results After the intervention,the level of resistance index and pul-satility index indicators of cerebral arteries in the observation group were lower than those of the control group,and the difference was statistically significant(both P<0.05).The mean blood flow velocity index level of the ob-servation group was(58.71±6.23)cm/s,which was higher than that of the control group(53.80±5.65)cm/s,and the difference was statistically significant(t=4.290,P<0.05).The National Institutes of Health Stroke Scale score of the observation group was lower than that of the control group,and the upper extremity function and lower extremity function scores were higher than those of the control group,and the difference was statistically significant(all P<0.05).Conclusion The addition of traditional Chinese medicine fumigation on the basis of conventional rehabilitation therapy and acupuncture treatment in cerebral infarction hemiplegia patients can lead to significant improvement of cerebral hemodynamic indexes,substantially reduce the degree of neurologi-cal function damage,and comprehensively improve the limb motor function.
