多层螺旋CT血管成像动态图像后处理对脑血管畸形病变的诊断价值研讨

摘要
目的 研究多层螺旋CT血管成像动态图像后处理运用于脑血管畸形病变患者中的效果.方法 随机选取2020年10月—2023年10月聊城市人民医院接诊的70例疑似脑血管畸形病变患者为研究对象,将数字减影血管造影结果作为诊断金标准,并且提供多层螺旋CT血管成像动态图像后处理,应用CT多平面重组、三维容积漫游及联合诊断分析诊断效能.结果 数字减影血管造影阳性检出率为48.57%,CT多平面重组诊断阳性检出率为42.86%,三维容积漫游诊断阳性检出率为44.29%,联合检查阳性检出率为51.43%.CT多平面重组诊断、三维容积漫游诊断的灵敏度、特异度、准确度相比,差异无统计学意义(P均>0.05).联合诊断的灵敏度、准确度显著高于CT多平面重组诊断,差异有统计学意义(P均<0.05).三维容积漫游诊断、联合诊断的灵敏度、特异度、准确度比较,差异无统计学意义(P均>0.05).结论 多层螺旋CT血管成像动态图像后处理中,CT多平面重组联合三维容积漫游诊断运用于脑血管畸形病变患者中具有较高的诊断效能,诊断价值高.
Abstract
Objective To study the effect of dynamic image post-processing of multilayer spiral CT angiog-raphy in patients with cerebrovascular malformation lesions.Methods Seventy patients with suspected cerebro-vascular malformation treated in Liaocheng People's Hospital from October 2020 to October 2023 were ran-domly selected as the study objects.The results of digital subtraction angiography were used as the diagnostic gold standard,and dynamic image processing of multi-slice spiral CT angiography was provided,application of CT multiplanar reconstruction,three-dimensional volume roaming and combined diagnosis to analyze diagno-sistic performance.Results The positive detection rate of digital subtraction angiography was 48.57%,the posi-tive detection rate of multi-plane CT recombination diagnosis was 42.86%,the positive detection rate of three-dimensional volume wandering diagnosis was 44.29%,and the positive detection rate of combined examination was 51.43%.There was no significant difference in sensitivity,specificity and accuracy between CT multi-plane recombination diagnosis and three-dimensional volume roaming diagnosis(all P>0.05).The sensitivity and accuracy of combined diagnosis were significantly higher than that of CT multi-plane recombination diag-nosis,and the differences were statistically significant(all P<0.05).There was no significant difference in sensi-tivity,specificity and accuracy between 3D volume roaming diagnosis and combined diagnosis(all P>0.05).Conclusion In the dynamic image post-processing of multi-slice spiral CT angiography,CT multi-planar re-construction combined with three-dimensional volume roaming diagnosis has high diagnostic efficiency and high diagnostic value in patients with cerebral vascular malformation.
