髓内钉治疗老年不稳定股骨转子间骨折的临床效果分析

摘要
目的 分析髓内钉治疗不稳定型股骨转子间骨折老年患者的临床效果.方法 随机选取2020年1月—2022年1月在山东省沂南县人民医院诊断的股骨转子间骨折稳定型的100例老年患者为研究对象.根据固定方式不同分为A组(50例,钛防旋髓内钉固定)及B组(50例,动力髋螺钉固定).对比两组患者手术过程情况及愈合时间,髋关节功能以及并发症.结果 两组手术时间比较,差异无统计学意义(P>0.05).A组住院时间、愈合时间对比B组明显缩短,A组髋关节功能优良率明显优于B组,差异有统计学意义(P均<0.05).A组(4.00%)并发症明显低于B组(16.00%),差异有统计学意义(χ2=4.000,P<0.05).结论 防旋髓内钉治疗不稳定股骨转子间骨折老年患者具有住院天数少,愈合时间短,骨折复位质量高的特点.
Abstract
Objective To analyze the clinical effect of intramedullary nail in the treatment of unstable in-tertrochanteric femoral fractures in the elderly.Methods A total of 100 elderly patients with unstable femoral intertrochanteric fracture diagnosed in Yinan County People's Hospital of Shandong Province from January 2020 to January 2022 were randomly selected as the study objects.According to different fixation methods,they were divided into group A(50 cases)with titanium anti rotation intramedullary nail fixation and group B(50 cases)with dynamic hip screw fixation.The operation process and healing time,hip function and complications were compared between the two groups.Results There was no statistically significant difference in operation time between group A and group B(P>0.05).And the length of hospital stay and healing time in group A were significantly shorter than those in group B,the excellent and good rate of hip function in group A was signifi-cantly better than that in group B,and the differences were statistically significant(all P<0.05).The complica-tions in group A(4.00%)were significantly lower than those in group B(16.00%),and the difference was statis-tically significant(χ2=4.000,P<0.05).Conclusion Intramedullary nail in the treatment of unstable intertro-chanteric femoral fractures in the elderly has the characteristics of fewer hospital days,shorter healing time and higher fracture reduction quality.
