多参数磁共振成像联合肝脏影像报告和数据管理系统分级对小肝癌患者的早期筛查诊断效果分析

摘要
目的 分析多参数磁共振成像(Multiparameter Magnetic Resonance Imaging,Mp-MRI)联合肝脏影像报告和数据管理系统(Liver Imaging Reporting and Data System,LI-RADS)分级对小肝癌患者的早期筛查诊断效果.方法 方便选择2021年10月—2022年10月邹城市人民医院收治的病灶≤3 cm的119例肝区占位性病变患者为研究对象,进行Mp-MRI检查及LI-RADS分级评估,以病理诊断结果为金标准,分析Mp-MRI、LI-RADS分级单独及联合诊断对小肝癌患者的早期筛查诊断效果.结果 病理诊断结果显示119例肝区占位性病变患者中84例为恶性小肝癌,35例为良性病变.Mp-MRI、LI-RADS分级联合诊断的灵敏度97.62%(82/84)、准确度97.48%(116/119)、阴性预测值94.44%(34/35),均高于两者单独诊断,差异有统计学意义(χ2=7.539、10.337、8.590,P均<0.05).结论 Mp-MRI、LI-RADS分级均对小肝癌患者具有一定的早期筛查效果,且两者联合可以提升诊断的准确性.
Abstract
Objective To analyze the effect of multiparameter magnetic resonance imaging(Mp-MRI)combined with liver image reporting and data system(LI-RADS)grading on early screening and diagnosis of small liver cancer patients.Methods A total of one hundred and nineteen patients with hepatic space occupy-ing lesions with lesions≤3 cm in Zoucheng People's Hospital from October 2021 to October 2022 were conve-niently selected as the study objects for Mp-MRI examination and LI-RADS grading evaluation.Using patho-logical diagnosis results as the gold standard,the early screening and diagnosis effects of Mp-MRI and LI-RADS classification alone and combined diagnosis on small liver cancer patients were analyzed.Results The pathological diagnosis results showed that among 119 patients with liver mass lesions,84 were diagnosed as ma-lignant small liver cancer and 35 were diagnosed as benign lesions.Joint diagnosis of Mp-MRI and LI-RADS graded diagnosis of small liver cancer had the sensitivity of 97.62%(82/84),accuracy of 97.48%(116/119),negative predictive value of 94.44%(34/36),which were higher than those of both single diagnosis,the differ-ences were statistically significant(χ2=7.539,10.337,8.590,all P<0.05).Conclusion Both Mp-MRI and LI-RADS grading have certain early screening effect on small liver cancer patients,and the combination of the two can improve the diagnostic accuracy.
