脑电图检查在偏头痛和头痛性癫痫诊断中的价值探讨

摘要
目的 比较常规脑电图和24 h动态脑电图检查在偏头痛和头痛性癫痫诊断中的价值.方法 随机选取2022年1月—2023年1月湖北省公安县中医医院收治的80例疑似偏头痛和头痛性癫痫患者为研究对象,均实施常规脑电图、24 h动态脑电图检查,对比不同疾病检出率,分析各个方案诊断效能.结果 以综合诊断结果为金标准,常规脑电图异常率为80.00%,24 h动态脑电图异常率为 97.14%.常规脑电图灵敏度(65.71%)、准确度(78.75%)均低于 24 h动态脑电图(94.28%、96.25%),差异有统计学意义(χ2=8.928、11.220,P均<0.05).常规脑电图特异度与24 h动态脑电图对比,差异无统计学意义(P>0.05).结论 在偏头痛和头痛性癫痫诊断中应用24 h动态脑电图不仅可准确鉴别疾病,同时可明确患者病情,为临床医师制订治疗方案提供参考.
Abstract
Objective To explore the value of electroencephalogram in the diagnosis of migraine and head-ache epilepsy.Methods A data of eighty patients with suspected migraine and headache epilepsy admitted to Gong'an County Hospital of Traditional Chinese Medicine in Hubei Province from January 2022 to January 2023 were randomly selected.Routine electroencephalogram and 24 h ambulatory electroencephalogram were examined.The detection rate of different diseases was compared,and the diagnostic efficiency of each scheme was analyzed.Results Using the comprehensive diagnosis as the gold standard,the abnormal rate of routine electroencephalogram was 80.00%,and the abnormal rate of 24 h ambulatory electroencephalogram was 97.14%.The sensitivity(65.71%)and accuracy(78.75%)of conventional electroencephalogram were lower than 94.28%and 96.25%of 24 h ambulatory electroencephalogram,the differences were statistically signifi-cant(χ2=8.928,11.220,both P<0.05).There was no statistically significant difference in the specificity be-tween conventional electroencephalogram and 24 h ambulatory electroencephalogram(P>0.05).Conclusion The application of 24 h ambulatory electroencephalogram in the diagnosis of migraine and headache epilepsy can not only accurately identify the disease,but also clarify the patient's condition,and provide references for clinicians to make treatment plans.
