四维超声在胎儿产前筛查唇腭裂中的准确度和灵敏度分析

摘要
目的 分析胎儿产前筛查唇腭裂中开展四维超声检查的准确度和灵敏度.方法 随机选取2021年4月—2022年12月在睢宁县妇幼保健院进行规律产检的2 000例孕妇作为研究对象,所有孕妇均接受二维超声及四维超声检查,以产前染色体核型分析或基因芯片检测结果作为金标准,采用受试者操作特征(Receiver Operating Characteristic,ROC)曲线评估二维超声及四维超声的诊断效能.结果 经引产或产后明确诊断12例胎儿属于唇腭裂.与二维超声相比,四维超声唇腭裂诊断准确率大幅提升,差异有统计学意义(P<0.05).四维超声Ⅱ度、Ⅲ度唇腭裂检出率均较二维超声明显更高,差异有统计学意义(P均<0.05).二维超声及四维超声诊断的ROC曲线下面积分别是0.847(95%CI:0.814~0.904,P<0.05)、0.915(95%CI:0.901~0.996,P<0.05).结论 胎儿产前筛查唇腭裂中开展四维超声检查比常规二维超声检查具有更高的准确度,能够实时、清晰、直观显示胎儿面部状况,辅助临床医师判断唇腭裂畸形程度,是一项安全可靠的技术.
Abstract
Objective To analyze the accuracy and sensitivity of 4-D ultrasound in prenatal screening for cleft lip and palate.Methods A total of two thousand pregnant women in Suining County Maternal and Child Health Hospital from April 2021 to December 2022 were randomly selected as the study subjects.All of them received 2-D ultrasound and 4-D ultrasound.The diagnostic efficacy of 2-D and 4-D ultrasound was evalu-ated by receiver operating characteristic(ROC)curve using prenatal chromosomal karyotype analysis or gene chip test results as the gold standard.Results A total of 12 fetuses were diagnosed with cleft lip and palate after labor induction or postpartum.Compared with 2-D ultrasound,the diagnosis accuracy of cleft lip and palate by 4-D ultrasound was significantly improved,and the difference was statistically significant(P<0.05).The detec-tion rates of Ⅱdegree and Ⅲ degree cleft lip and palate by 4-D ultrasound were significantly higher than those by 2-D ultrasound,and the difference was statistically significant(both P<0.05).The area under ROC curve of 2-D and 4-D ultrasound diagnosis was 0.847(95%CI=0.814-0.904,P<0.05)and 0.915(95%CI=0.901-0.996,P<0.05),respectively.Conclusion In prenatal screening for cleft lip and palate,4-D ultrasound has higher ac-curacy than conventional 2-D ultrasound,which can display the facial condition of the fetus in real time,clearly and visually,and assist clinicians to judge the degree of cleft lip and palate deformity.It is a safe and re-liable technology.
