血府逐瘀汤加减治疗冠心病稳定型心绞痛临床疗效及对左心室功能的影响

摘要
目的 评估血府逐瘀汤加减对冠状动脉粥样硬化性心脏病(简称冠心病)稳定型心绞痛气滞血瘀证患者的临床疗效、左心室功能的影响.方法 随机选取2022年10月—2023年6月济宁市兖州区人民医院急诊医学科收治的100例稳定型心绞痛患者为研究对象,按照随机对照方法分为治疗组和对照组,每组50例.对照组仅接受常规西药治疗,治疗组在对照组治疗的基础上加用血府逐瘀汤加减,治疗1个月后比较两组临床疗效、血清同型半胱氨酸(Homocysteine,Hcy)水平、中医证候评分、左心室功能指标变化以及不良反应发生情况.结果 治疗组的临床总有效率(93.75%)高于对照组(61.22%),差异有统计学意义(χ2=14.627,P<0.05).治疗后,治疗组的中医证候评分、Hcy水平、左室舒张末期容积和左室收缩末期容积水平均低于对照组,差异有统计学意义(P均<0.05);左室射血分数高于对照组,差异有统计学意义(P<0.05);两组均未发现明显不良反应.结论 血府逐瘀汤加减治疗冠心病稳定型心绞痛临床疗效可靠,可降低血清Hcy水平,并能改善左心室功能.
Abstract
Objective To evaluate the clinical efficacy and the effect of Xuefu Zhuyu decoction plus reduc-tion on the left ventricular function of patients with stable angina pectoris and Qi stagnation and blood stasis syndrome in coronary artery atherosclerotic heart disease(hereinafter referred to as coronary heart disease).Methods A total of 100 patients with stable angina pectoris admitted to the Emergency Medicine Department of Yanzhou District People's Hospital of Jining from October 2022 to June 2023 were randomly selected as the re-search objects.According to the randomized controlled method,they were divided into the treatment group and the control group,with 50 cases in each group.The control group was treated with conventional western medi-cine,and the treatment group was treated with Xuefu Zhuyu decoction on the basis of the control group.After 1 month of treatment,the clinical efficacy,serum homocysteine(Hcy)level,traditional Chinese medicine syn-drome score,left ventricular function index changes and adverse reactions were compared between the two groups.Results The total effective rate of the treatment group(93.75%)was higher than that of the control group(61.22%),and the difference was statistically significant(χ2=14.627,P<0.05).After treatment,the traditional Chinese medicine symptom score,Hcy level,left ventricular end-diastolic volume and left ventricular end-systolic volume levels in the treatment group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The left ventricular ejection fraction was higher than that of the con-trol group,and the difference was statistically significant(P<0.05).No obvious adverse reactions were found in the two groups.Conclusion The treatment of stable angina pectoris in coronary artery disease with Xuefu Zhuyu decoction plus reduction is clinically effective and reliable,which can reduce the serum Hcy level and improve the left ventricular function.
