摘要

目的 评估重症肺炎(Severe Pneumonia,SP)患者接受纤维支气管镜下支气管肺泡灌洗术(Bronchoalveolar Lavage,BAL)的价值.方法 随机选取2019年6月—2022年6月赤壁市蒲纺医院收治的70例SP患者为研究对象,根据随机数表法分为对照组和观察组,各35例.对照组给予亚胺培南西司他丁钠治疗,观察组给予亚胺培南西司他丁钠+BAL治疗.对比两组治疗有效率、炎性因子、血气指标、量表评分、安全性.结果 观察组治疗有效率为94.29%,高于对照组的77.14%,差异有统计学意义(χ2=4.200,P<0.05).治疗前,两组炎性因子、血气指标、量表评分对比,差异无统计学意义(P均>0.05).治疗1周后,观察组炎性因子、血气指标、量表评分均高于对照组,差异有统计学意义(P均<0.05).两组不良反应比较,差异无统计学意义(P>0.05).结论 对SP患者实施BAL,可以提高治疗有效率,改善缺氧症状及通气功能,控制肺部炎症,缓解病情,减轻肺组织损伤,安全可靠.

Abstract

Objective To evaluate the value of bronchoalveolar lavage(BAL)under fiberoptic bronchos-copy in patients with severe pneumonia(SP).Methods A total of 70 patients with SP in Pufang Hospital of Chibi City from June 2019 to June 2022 were randomly selected as the study objects,and were divided into con-trol group and observation group according to random number table method,with 35 cases in each group.The control group was treated with imipenem and cilastatin sodium,and the observation group was treated with imi-penem and cilastatin sodium plus BAL.The therapeutic efficacy,inflammatory factors,blood gas indexes,scale scores and safety of the two groups were compared.Results The effective rate of the observation group was 94.29%,which was higher than that of the control group(77.14%),the difference was statistically significant(χ2=4.200,P<0.05).Before treatment,there were no significant differences in inflammatory factors,blood gas in-dexes and scale scores between the two groups(all P>0.05).After 1 week of treatment,the inflammatory factors,blood gas indexes and scale scores of the observation group were higher than those of the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The implementation of BAL for SP patients can improve the treat-ment efficiency,improve hypoxia symptoms and ventilation function,control lung inflammation,alleviate the condition,reduce lung tissue damage,safe and reliable.
