摘要
目的 探讨对消化道早癌患者采用内镜黏膜下剥离术(Endoscopic Submucosal Dissection,ESD)治疗的临床效果.方法 回顾性选取2015年1月—2023年9月天等县人民医院收治的250例消化道早癌患者为研究对象展开分组对比研究,依据为投掷硬币法分为两组,各125例.对照组采用内镜黏膜切除术(Endoscopic Mucosal Resection,EMR)治疗,研究组采用ESD治疗.比较两组患者并发症总发生率、术中失血量、创面愈合时间、平均住院时间、完整切除率、生活质量评分.结果 研究组并发症总发生率(1.60%)低于对照组(8.00%),差异有统计学意义(χ2=5.602,P<0.05.研究组术中失血量、创面愈合时间、平均住院时间、完整切除率、生活质量评分均优于对照组,差异有统计学意义(P均<0.05).结论 临床对消化道早癌患者合理展开ESD治疗,同EMR比较,可显著提高患者安全性,同时呈现出恢复快以及出血少的优势,可显著改善患者生活质量,降低复发风险.
Abstract
Objective To explore the clinical effect of endoscopic submucosal dissection(ESD)for early cancer of the gastrointestinal tract after completing the treatment.Methods Retrospective selection 250 pa-tients with early gastrointestinal cancer admitted to Tiantan County People's Hospital from January 2015 to September 2023 as the research objects to carry out a group comparative study.They were divided into two groups according to coin tossing method,125 cases in each group.The reference group was treated with endo-scopic mucosal resection(EMR).The study group was treated with ESD.The total incidence of complications,intraoperative blood loss,wound healing time,average length of hospital stay,complete resection rate,and qual-ity of life score were compared between the two groups.Results The total incidence of complications in the study group(1.60%)was lower than that in the reference group(8.00%),and the difference was statistically sig-nificant(χ2=5.602,P<0.05).The intraoperative blood loss,wound healing time,average hospitalization time,complete resection rate,and quality of life score in the study group were better than those in the reference group,and the differences were statistically significant(all P<0.05).Conclusion Compared with EMR treat-ment,reasonable application of ESD in patients with early gastrointestinal cancer can significantly improve the safety of patients,show the advantages of rapid recovery and less bleeding,significantly improve the quality of life of patients,reduce the risk of recurrence,and fully prove the value of endoscopic submucosal dissection for patients with early gastrointestinal cancer.
