能谱CT血管造影对壶腹部周围癌及其术前分期的诊断价值研究

摘要
目的 分析能谱CT血管造影对壶腹部周围癌及其术前分期的诊断价值.方法 方便选取2021年2月—2023年8月山东省立医院收治的88例疑似壶腹部周围癌患者作为研究对象,对其展开常规增强CT扫描检查后,再进行能谱CT血管造影检查,以病理诊断结果作为金标准,观察两种检查诊断方式的诊断结果、对壶腹部周围癌的诊断效能及术前分期诊断结果符合率.结果 病理诊断结果显示阳性病灶和阴性病灶分别为70例和18例,常规增强CT扫描诊断为58例和30例,能谱CT血管造影检查诊断为70例和18例.能谱CT血管造影检查诊断灵敏度(97.14%)、特异度(88.89%)、准确度(95.45%)明显高于常规增强CT扫描(71.43%、55.56%、68.18%),差异有统计学意义(χ2=17.473、4.985、22.000,P均<0.05).能谱CT血管造影检查术前T2期和T3期的诊断结果符合率明显高于常规增强CT扫描,差异有统计学意义(χ2=5.439、4.615,P均<0.05).结论 在壶腹部周围癌患者的临床检查诊断中,能谱CT血管造影诊断价值高,不仅可以及时发现患者的阳性病灶,而且对患者术前T分期判断也可以带来一定指导作用,对临床治疗方案的制订提供一定的科学依据.
Abstract
Objective To analyze the diagnostic value of energy spectrum CT angiography on periampul-lary carcinoma and preoperative staging.Methods 88 patients with suspected periampullary carcinoma admit-ted to Shandong Provincial Hospital from February 2021 to August 2023 were conveniently selected as study ob-jects,and were examined by conventional enhanced CT scanning and then underwent energy spectrum CT angi-ography.The diagnostic results of the two modes of examination and the diagnostic efficacy of different modes of examination in diagnosing periampullary carcinoma were observed using the pathological diagnostic results as the gold standard.At the same time,the conformity rate of the preoperative staging diagnostic results of periam-pullary carcinoma by means of different examinations was observed.Results Pathologic diagnosis showed that there were 70 cases of positive lesions and 18 cases of negative lesions,58 cases of positive lesions and 30 cases of negative lesions were diagnosed by conventional enhanced CT scanning,and 70 cases of positive lesions and 18 cases of negative lesions were diagnosed by spectral CT angiography.The diagnostic sensitivity(97.14%),specificity(88.89%),and accuracy(95.45%)of energy spectrum CT angiography were higher than those of con-ventionally enhanced CT scanning(71.43%,55.56%and 68.18%),the differences were statistically significant(χ2=17.473,4.985,22.000,all P<0.05).The compliance rate of diagnostic results of preoperative T2 and T3 stages in energy spectrum CT angiography was higher than that of conventional enhanced CT scanning,the dif-ferences were statistically significant(χ2=5.439,4.615,both P<0.05).Conclusion In the clinical examination and diagnosis of patients with periampullary carcinoma of jugular abdomen,the diagnostic value of spectral CT angiography is high,which can not only timely detect the positive lesions of the patients,but also bring certain guidance to the judgment of preoperative T staging of the patients,and bring certain scientific basis for the for-mulation of clinical treatment plan.
