DWI、T2WI、ADC图多序列影像组学在直肠癌淋巴结转移预测中的应用分析

摘要
目的 探讨弥散加权成像(diffusion weighted imaging,DWI)、T2加权图像(T2-weight imag-ing,T2WI)、表观弥散系数(apparent dispersion coefficient,ADC)图多序列影像组学预测直肠癌淋巴结转移的价值.方法 选取2021年1月—2023年6月沭阳县中医院收治的62例直肠癌患者为研究对象,采用DWI、T2WI、ADC评价淋巴结转移情况,以病理检查结果为金标准,以受试者操作特征(re-ceiver operating characteristic,ROC)曲线评价DWI、T2WI、ADC对淋巴结转移情况的诊断效能.结果 病理检查结果为:无淋巴结转移35例,淋巴结转移27例.单独使用DWI模型:无淋巴结转移34个、淋巴结转移28个,T2WI模型:无淋巴结转移37个、淋巴结转移25个,ADC模型:无淋巴结转移40个、淋巴结转移22个,多序列影像组学:无淋巴结转移35个、淋巴结转移27个.经ROC曲线分析,T2WI模型预测淋巴结转移的AUC为0.709,灵敏度为74.07%,特异度为85.71%;DWI分别为0.863、81.48%、82.86%,ADC模型依次为0.817、70.37%、91.43%,多序列模型预测效能依次为0.973、92.59%、94.29%.结论 对于直肠癌淋巴结转移,DWI、T2WI、ADC图多序列影像组学模型的诊断效能较高.
Abstract
Objective To analyze the effect of diffusion weighted imaging(DWI),T2-weighted images(T2WI)and apparent diffusion coefficient(ADC)images on the prediction of lymph node metastasis in rectal cancer.Methods A total of 62 patients with rectal cancer admitted to Shuyang County Hospital of Traditional Chinese Medicine from January 2021 to June 2023 were selected as the study objects.DWI,T2WI and ADC were used to evaluate lymph node metastasis.Pathological examination results were taken as the gold standard,and receiver operating characteristic curve was used to evaluate the diagnostic efficiency of each model for lymph node metastasis.Results The pathological examination results showed that there were 35 cases without lymph node metastasis and 27 cases with lymph node metastasis.Using DWI model alone:34 without lymph node metastasis and 28 with lymph node metastasis;T2WI model:37 without lymph node metastasis and 25 with lymph node metastasis;ADC model:40 without lymph node metastasis and 22 with lymph node metastasis;multi sequence imaging omics:35 without lymph node metastasis and 27 with lymph node metastasis.Accord-ing to ROC curve analysis,the AUC of the T2WI model for predicting lymph node metastasis is 0.709,with a sensitivity of 74.07%and a specificity of 85.71%;The DWI values were 0.863,81.48%,and 82.86%,respec-tively.The ADC model values were 0.817,70.37%,and 91.43%,respectively.The predictive performance of the multi sequence model values were 0.973,92.59%,and 94.29%,respectively.Conclusion For the lymph nodes metastasis of rectal cancer,DWI,T2WI,ADC multi-sequence imaging model has higher diagnostic effi-ciency.
