手术配合大黄牡丹汤加减治疗化脓性阑尾炎的临床效果

摘要
目的 探究化脓性阑尾炎患者采用外科手术联合大黄牡丹汤加减治疗的效果.方法 方便选取2022年3月—2023年3月枣庄市薛城区中医院收治的78例化脓性阑尾炎患者为研究对象,以摸球法划分为对照组和中医组,各39例.对照组执行手术治疗,中医组执行手术配合大黄牡丹汤加减治疗,对比两组恢复情况.结果 中医组治疗总有效率(97.44%)高于对照组(76.92%),差异有统计学意义(χ2=7.341,P<0.05).中医组并发症发生率(切口感染、肠粘连和肺部感染)低于对照组,差异有统计学意义(P均<0.05).与对照组相比,中医组排气时间、肠鸣音恢复时间、通便时间、住院时间更短,差异有统计学意义(P均<0.05).治疗后中医组的降钙素原、C反应蛋白、白细胞介素-6水平均低于对照组,差异有统计学意义(P均<0.05).结论 在化脓性阑尾炎的临床治疗中,手术配合大黄牡丹汤加减治疗具有较好的效果,值得临床借鉴.
Abstract
Objective To investigate the efficacy of combined treatment with Dahuang Mudan decoction plus reduction on the basis of providing surgical treatment for patients with septic appendicitis.Methods Seventy-eight patients with purulent appendicitis disease admitted to Zaozhuang Xuecheng District Hospital of Traditional Chinese Medicine were conveniently selected from March 2022 to March 2023,and they were di-vided into the control group and the traditional chinese medicine group by the touch-ball method,with 39 cases in each group.The control group performed surgical treatment,and the traditional chinese medicine group per-formed surgical treatment with rhubarb and peony soup plus reduction to compare the recovery in the two groups.Results The treatment total effectiveness rate of the traditional Chinese medicine group(97.44%)was higher than that of the control group(76.92%),and the difference was statistically significant(χ2=7.341,P<0.05).The incidence of complications(incision infection,intestinal adhesion and lung infection)were lower in the traditional Chinese medicine group than in the control group,and the differences were statistically signifi-cant(all P<0.05).Compared with the control group,the time of gas evacuation,time of bowel sound recovery,time of laxation and hospitalization time were shorter than those in the traditional Chinese medicine group,and the differences were statistically significant(all P<0.05).The levels of calcitoninogen,C-reactive protein,and interleukin-6 were lower in the traditional Chinese medicine group after treatment than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion In the clinical treatment of pu-rulent appendicitis,surgery with rhubarb-mudan soup plus reduction treatment has better effect,which is wor-thy of clinical reference.
