摘要

目的比较钢板固定与髓内钉固定用于Masquelet技术治疗成人下肢长骨慢性骨髓炎的疗效.方法随机选取2022年1-6月聊城市传染病医院收治的60例成人下肢长骨慢性骨髓炎患者作为研究对象,根据Masquelet技术中第二期诱导膜内植骨固定方式的不同,将其分为髓内钉固定组(n=30)与钢板固定组(n=30).对比两组患者的相关指标.结果髓内钉固定组愈合时间及负重时间短于钢板固定组,差异有统计学意义(P均<0.05).髓内钉固定组并发症总发生率为3.33%,低于钢板固定组的10.00%,但差异无统计学意义(P>0.05).治疗前,两组患者生活质量水平对比,差异无统计学意义(P>0.05);治疗后,髓内钉固定组生活质量水平高于钢板固定组,差异有统计学意义(P<0.05).结论与钢板固定相比,髓内钉固定有助于提高成人下肢长骨慢性骨髓炎患者生活质量水平,促进患者术后恢复,临床疗效显著.

Abstract

Objective To compare the efficacy of plate fixation and intramedullary nail fixation in the treat-ment of chronic osteomyelitis of the long bones of the adult lower limbs using the Masquelet technique.Meth-ods A total of 60 adult patients with chronic osteomyelitis of long bones of lower limbs treated in Liaocheng In-fectious Disease Hospital from January to June 2022 were randomly selected as the research objects.According to different methods of second-stage intramedullage bone grafting induced by Masquelet,these patients were di-vided into intramedullage nailing group(n=30)and plate fixing group(n=30).The relevant indexes of the two groups were compared.Results The healing time and weight-bearing time of intramedullary nail fixation group were shorter than those of plate fixation group,and the differences were statistically significant(both P<0.05).The total complication rate of intramedullary nail fixation group was 3.33%,lower than that of plate fixation group(10.00%),and the difference was not statistically significant(P>0.05).Before treatment,the quality of life levels of patients in the two groups were compared,and the difference was not statistically significant(P>0.05).After treatment,the quality of life level in the intramedullary nail fixation group was higher than that in the plate fixation group,and the difference was statistically significant(P<0.05).Conclusion Compared with plate fixing group,intramedullary nail fixation helps improve chronic osteomyelitis of adult lower limb long bones patients'quality of life and promotes postoperative recovery.It has significant clinical effects.
