Objective To compare the effects of total hip replacement and bipolar artificial femoral head re-placement in the treatment of elderly femoral neck fractures. Methods A total of 90 elderly patients with femo-ral neck fracture treated in Shouguang People's Hospital of Shandong Province from January 2021 to October 2023 were simply and randomly selected as the study subjects, the patients were divided into two groups by blind selection and drawing method. The control group(45 cases)underwent bipolar artificial femoral head re-placement and the study group(45 cases)underwent total hip replacement. Results The intraoperative blood loss and postoperative drainage volume were more in the study group than in the control group, and the length of surgery and hospitalization were shorter than in the control group, and the differences were statistically signifi-cant(all P<0.05).One year after surgery, the hip function scores of the study group were higher than those of the control group, and the difference was statistically significant(all P<0.05).The total complication rate of the study group(6.67%)was similar to that of the control group(8.89%),and the difference was not statistically sig-nificant(χ2=0.155,P=0.694).Conclusion Total hip arthroplasty and bipolar prosthetic femoral head replace-ment have high safety in the treatment of elderly femoral neck fractures respectively.Bipolar prosthetic femoral head replacement can shorten the postoperative recovery time, but total hip arthroplasty has better functional re-covery.
